What Is the Best Viagra: How the 4 Options Differ

There is no single “best” ED medication. The four FDA-approved options all belong to the same drug class and produce successful intercourse in 70 to 75% of attempts across broad patient populations. The real differences come down to how fast they work, how long they last, and how your body tolerates them. The best choice depends on your lifestyle, how often you have sex, and whether you have other health conditions that narrow your options.

How the Four Options Compare

All four medications work the same way: they increase blood flow to the penis by relaxing smooth muscle tissue. They don’t create arousal on their own, and they all require sexual stimulation to work. Where they differ is timing.

  • Sildenafil (Viagra): Works in 30 to 60 minutes, lasts up to 12 hours. A high-fat meal delays absorption and reduces effectiveness.
  • Tadalafil (Cialis): Works in 60 to 120 minutes, lasts up to 36 hours. Food does not affect absorption.
  • Vardenafil (Levitra): Works in 30 to 60 minutes, lasts up to 10 hours. A high-fat meal can delay absorption by about an hour.
  • Avanafil (Stendra): Works in 15 to 30 minutes, lasts up to 6 hours. Food does not affect absorption.

In head-to-head comparisons, sildenafil, tadalafil, and vardenafil show similar efficacy in the general ED population. The American Urological Association does not rank one above the others. Instead, clinical guidelines emphasize matching the medication to the patient’s goals and lifestyle.

When Duration Matters Most

Tadalafil stands apart because of its 36-hour window, sometimes called the “weekend pill.” That long duration means you don’t have to time a dose right before sex. You can take it Friday evening and still have it working Sunday morning. This removes the pressure of planning and makes sex feel more spontaneous, which is a major reason many men and their partners prefer it.

Tadalafil also comes in a daily low-dose option (2.5 or 5 mg taken every day) rather than an as-needed dose. The daily version maintains a steady level in your system so the medication is always active. Some research suggests that partners tend to prefer the daily approach over the on-demand model, likely because it removes the medication from the moment entirely. The daily version is also FDA-approved to treat an enlarged prostate, so men dealing with both conditions can address them with one pill.

When Speed Matters Most

Avanafil is the fastest-acting option, reaching peak levels in as little as 15 to 30 minutes. If you want something that works quickly without much advance planning, this is the strongest choice on timing alone. It also appears to cause fewer side effects. In one clinical trial comparing avanafil to sildenafil in men recovering from prostate surgery, only 5% of the avanafil group experienced side effects compared to 20% in the sildenafil group.

The trade-off is that avanafil has the shortest window of action at roughly 3 to 6 hours. If you need flexibility across a longer evening or weekend, tadalafil is the better fit.

Side Effects Are Similar Across the Board

All four medications share the same common side effects: headache, facial flushing, nasal congestion, and upset stomach. Published reviews have found that these effects are similarly frequent, similarly mild, and similarly dose-dependent for all the major options. Higher doses produce more side effects across the board.

Tadalafil does have one notable addition. Because of its longer duration, it can cause muscle aches and back pain. In clinical trials, over 10% of men taking tadalafil on demand reported these symptoms. At higher doses in early trials, discontinuation rates due to side effects climbed steeply: 19% of patients stopped at 50 mg and 29% stopped at 100 mg, though approved doses today max out at 20 mg, which is much better tolerated.

Sildenafil can cause a temporary blue-tinged vision in some men, a side effect less common with the other three.

Food and Alcohol Interactions

This is a practical detail that trips people up. Sildenafil and vardenafil both lose effectiveness when taken with a heavy meal. A high-fat dinner can delay vardenafil’s absorption by a full hour and reduce how much of the drug actually reaches your bloodstream. A moderate-fat meal has minimal impact, but if you’re eating a steak dinner before a date, the timing may not work as expected.

Tadalafil and avanafil are unaffected by food, which makes them more forgiving if you’re taking a dose around a meal. If you tend to use ED medication on date nights that involve dinner, this distinction matters more than most people realize.

Cost Differences Are Dramatic

Generic sildenafil is by far the cheapest option. It can cost under 50 cents per dose when purchased in quantity, compared to brand-name Viagra at over $67 per pill. That price gap makes generic sildenafil the default starting point for most men, and it’s one reason sildenafil remains the most widely prescribed ED medication despite newer alternatives.

Generic tadalafil is also available and significantly cheaper than brand-name Cialis, though typically still more expensive per pill than generic sildenafil. Avanafil tends to be the priciest because it’s newer and has less generic competition. If cost is a factor, starting with generic sildenafil or generic tadalafil covers most situations well.

One Critical Safety Rule

None of these medications can be taken with nitrate-based heart medications, including nitroglycerin tablets or patches. The combination causes a dangerous drop in blood pressure. The American Heart Association specifies that nitrates can only be used safely at least 24 hours after a fast-acting option like sildenafil and at least 48 hours after tadalafil, given its longer duration. If you take nitrates for chest pain or a heart condition, these medications are off the table entirely without a cardiologist’s involvement.

Choosing the Right One for You

The success rates are nearly identical, so the decision comes down to lifestyle fit. If you want the cheapest, most widely available option and don’t mind timing it around meals, generic sildenafil is the practical default. If you want a longer window and more spontaneity, tadalafil (especially the daily low-dose version) is the most popular alternative. If speed and fewer side effects are your priority, avanafil is worth considering despite the higher cost.

Many men try more than one before settling on a preference. A medication that doesn’t work well on the first attempt may work on the second or third try, and switching to a different option is common. Success rates drop to 40 to 50% in men with nerve-related causes like diabetes or prior pelvic surgery, so expectations should be adjusted in those situations.
