Is Wegovy an Injection or a Pill? Here’s the Answer

Yes, Wegovy is a once-weekly injection given just under the skin. It comes as a prefilled pen with a hidden, built-in needle, so there’s no assembly or priming required. You press the pen against your skin, and the device delivers the dose automatically in about 5 to 10 seconds.

How the Pen Works

Each Wegovy pen is single-use and pre-loaded with your prescribed dose. The needle is concealed inside the device, which makes the process straightforward even if you’ve never given yourself an injection before. After cleaning the injection site, you pull off the pen cap, press the pen firmly against your skin, and hold it in place. A yellow bar in the pen window moves as the medication is delivered, and you’ll hear two clicks during the process. Once the yellow bar stops moving, the injection is complete.

You can inject into three areas: the front of your thigh, your lower stomach (at least two inches from your belly button), or your upper arm. Rotating between these sites each week is important. Injecting in the same spot repeatedly can cause small nodules or changes in the tissue underneath the skin, which may interfere with how well the medication is absorbed.

Injection Frequency and Dose Schedule

Wegovy is injected once a week, on the same day each week, at any time of day, with or without food. The dose starts low and gradually increases over about four months to give your body time to adjust and reduce the chance of nausea and other stomach-related side effects.

The standard schedule looks like this:

  • Weeks 1 through 4: 0.25 mg
  • Weeks 5 through 8: 0.5 mg
  • Weeks 9 through 12: 1 mg
  • Weeks 13 through 16: 1.7 mg
  • Week 17 onward: 1.7 mg or 2.4 mg for adults, 2.4 mg for adolescents

Each dose level uses a different pen, so your prescription changes as you move through the schedule.

Is There an Oral Version?

Wegovy is also now available as a tablet for some of its approved uses in adults. However, injectable semaglutide (the active ingredient in Wegovy) produces stronger weight loss results than oral formulations. The American College of Gastroenterology has noted that oral versions of this class of medication are less potent than their injectable counterparts. For most people prescribed Wegovy for weight management, the injection remains the standard form.

What Wegovy Is Approved For

The FDA has approved Wegovy for several uses. It can be prescribed for long-term weight management in adults with obesity, or in adults with overweight who also have at least one weight-related health condition such as high blood pressure, type 2 diabetes, or high cholesterol. It’s also approved for adolescents aged 12 and older with obesity. Beyond weight loss, Wegovy has a separate approval for reducing the risk of heart attack, stroke, and cardiovascular death in adults with established heart disease who also have obesity or overweight. More recently, it received accelerated approval for treating a form of fatty liver disease with moderate to advanced scarring.

Common Injection Site Reactions

More than 1% of people using Wegovy experience some kind of reaction at the injection site. The most common are redness, mild pain, or a slight rash. These reactions are generally mild and tend to improve on their own. In rare cases, small firm lumps can develop under the skin, particularly if you inject in the same spot each week. Rotating between your thigh, stomach, and upper arm helps prevent this.

Storing the Pens

Wegovy pens should be kept in the refrigerator between 36°F and 46°F, ideally in the original carton. If you’re traveling or don’t have access to a fridge, pens can stay at room temperature for up to 28 days as long as the temperature stays below 86°F and the pen cap hasn’t been removed. After 28 days out of the fridge, or if the pen has been frozen, exposed to direct light, or left somewhere warmer than 86°F, it should be thrown away. Used pens go in a sharps disposal container or a sturdy household container with a tight-fitting lid, not in regular trash or recycling.
