Bijuva is taken at night because its FDA-approved label specifically directs you to take one capsule each evening with food. This timing isn’t arbitrary. It aligns the medication’s peak effects with sleep, takes advantage of progesterone’s natural sedating properties, and targets night sweats during the hours they actually occur.

The Official Dosing Instructions

The FDA prescribing information for Bijuva states plainly: “Take 1 capsule by mouth each evening with food.” This isn’t a suggestion or a general guideline. Evening dosing is the way the drug was studied in clinical trials and the way it was approved. The “with food” part matters too, since food increases absorption of both hormones in the capsule.

Progesterone Causes Drowsiness

Bijuva combines estradiol and progesterone in a single capsule. The progesterone component is the main reason for nighttime dosing. Oral progesterone activates the same brain receptors that sleep-promoting and anti-anxiety medications target (called GABA receptors), which produces a calming, sedative effect. Progesterone in Bijuva reaches its peak blood levels roughly 2 to 3 hours after you take it, meaning the strongest sedative effect hits right around the time you’d want to be falling asleep.

Taking it in the morning would mean dealing with that drowsiness during the day, which could affect your alertness, concentration, and ability to drive safely. Evening dosing essentially turns a side effect into a benefit.

It Improves Sleep Quality

Many women taking Bijuva experience measurable improvements in sleep. In the large REPLENISH clinical trial, women taking the 1 mg estradiol/100 mg progesterone dose showed significant improvement in sleep quality scores over 12 months. Researchers attributed this to two factors working together: estradiol reducing the hot flashes and night sweats that wake women up, and progesterone directly promoting sleep through its effects on GABA receptors.

This dual effect makes nighttime dosing especially logical. The estradiol component takes longer to reach peak levels, around 6 to 10 hours after a dose on the first day, and roughly 5 to 6 hours with regular daily use. So if you take Bijuva at 9 p.m., estradiol levels peak during the early morning hours when night sweats tend to be most disruptive.

Timing Targets Night Sweats When They Happen

Vasomotor symptoms like hot flashes and night sweats are among the primary reasons Bijuva is prescribed. Night sweats commonly hit during the second half of the night and early morning. Because estradiol takes several hours to reach its highest levels in your blood, evening dosing means the medication is working hardest precisely when nighttime symptoms are most likely to occur. This is a more effective strategy than morning dosing, which would leave peak estradiol levels during the afternoon.

What to Do if You Miss a Dose

If you forget your evening dose, take it as soon as you remember, with food. The one exception: if it’s within two hours of your next scheduled evening dose, skip the missed one entirely and take the next dose at your regular time. Don’t double up to make up for a missed capsule.

Consistency matters with hormone therapy. Taking Bijuva at roughly the same time each evening helps maintain steady hormone levels and keeps the sleep-promoting effects predictable. Pairing it with a consistent part of your evening routine, like dinner or a bedtime snack, can make it easier to remember and ensures you’re always taking it with food as required.
