Amlodipine treats high blood pressure and chest pain (angina). While it offers significant cardiovascular benefits, many individuals experience a common side effect: swelling, particularly in the ankles and feet. How Amlodipine Works
Amlodipine is a calcium channel blocker. It functions by relaxing and widening blood vessels throughout the body. By inhibiting the influx of calcium ions into smooth muscle cells of blood vessel walls, amlodipine prevents these muscles from contracting. This vasodilation reduces the resistance against which the heart must pump blood, effectively lowering blood pressure and improving blood flow to the heart muscle, which helps alleviate angina. However, this same mechanism also plays a role in the fluid accumulation that can lead to swelling in some individuals.
The Mechanism Behind Swelling
The swelling associated with amlodipine results from its impact on the body’s small blood vessels. Amlodipine preferentially dilates arterioles (small arteries carrying blood away from the heart) more than venules (small veins returning blood to the heart). This uneven dilation creates a pressure imbalance within the capillary beds, forcing fluid to leak out and accumulate in surrounding tissues, a condition known as edema.
This fluid leakage is not typically a sign of heart or kidney failure, but rather a direct pharmacological effect of the medication. The swelling is commonly observed in the lower extremities, such as the ankles and feet, due to gravity. It often appears symmetrical, affecting both legs, and is generally painless. The risk and severity of this edema can increase with higher doses of amlodipine and with longer-term use.
Addressing Amlodipine-Induced Swelling
Managing amlodipine-induced swelling involves lifestyle adjustments and, if necessary, medical interventions. Elevating the legs above heart level, especially while resting, encourages fluid to drain away from the lower limbs. Wearing compression stockings provides support and prevents fluid from pooling in the ankles and feet.
Engaging in regular light exercise, such as walking, improves circulation and helps reduce swelling. While diuretics are generally not effective for this type of edema because it is a fluid redistribution issue rather than fluid retention, maintaining good hydration and reducing sodium intake can be beneficial. If these measures are insufficient, a doctor might consider adjusting the amlodipine dose or adding another medication, such as an ACE inhibitor or an angiotensin receptor blocker, which can help balance the pressure in the blood vessels and reduce swelling.

While amlodipine-induced swelling is common and often not harmful, certain signs warrant medical evaluation. Consult a healthcare provider if the swelling appears suddenly or becomes severe. Swelling that affects only one leg, or is accompanied by pain, redness, or warmth, may indicate a more serious condition like a deep vein thrombosis (blood clot).
Seek medical attention if the swelling spreads to other parts of the body, such as the face or abdomen, or if it is accompanied by shortness of breath, unexplained weight gain, or chest pain.
