The subscapularis muscle is a key component of the shoulder’s intricate muscular system. It is one of the four muscles that form the rotator cuff, a group essential for shoulder movement and stability. This muscle is located on the anterior aspect of the shoulder blade, known as the scapula.
Subscapularis Anatomy
The subscapularis muscle is a large, triangular-shaped muscle that fills the subscapular fossa, a broad depression on the front surface of the scapula. This fossa serves as its primary origin point, specifically the medial two-thirds of this area. From this origin, the muscle fibers extend laterally across the front of the shoulder joint.
These fibers converge to form a robust tendon that inserts precisely onto the lesser tubercle of the humerus, the upper arm bone. Some fibers also blend with the anterior part of the shoulder joint capsule.
The subscapularis is unique among the rotator cuff muscles because it is the only one situated on the anterior side of the scapula. The other three rotator cuff muscles (supraspinatus, infraspinatus, and teres minor) are located on the posterior side. This makes the subscapularis the largest and strongest of the four rotator cuff muscles.
Subscapularis Function
The subscapularis muscle plays a primary role in the movement of the arm. Its main action is internal rotation, or medial rotation, of the humerus at the shoulder joint. This movement involves rotating the arm inward, towards the body’s midline.
This muscle also contributes to shoulder adduction, bringing the arm closer to the torso. Beyond its role in movement, the subscapularis is important for maintaining the stability of the shoulder. It helps to keep the head of the humerus centered within the glenoid fossa, the shallow socket of the shoulder blade.
This stabilizing action is often referred to as concavity compression. By compressing the humeral head into the glenoid, the subscapularis works with the other rotator cuff muscles to prevent dislocations and ensure smooth movement across the joint. This coordinated effort allows for a wide range of motion while maintaining joint integrity.
Importance of the Subscapularis
