The human spine is made up of individual bones called vertebrae, stacked one upon another. The L4 and L5 vertebrae are located in the lumbar region, commonly known as the lower back. These two specific bones play a significant role in the spine’s overall function.
To locate L4 and L5, one can identify them within the lumbar curve, which is the natural inward curve of your lower back. The L4 vertebra sits directly above the L5 vertebra. The L5 vertebra is the lowest of the lumbar vertebrae, positioned immediately above the sacrum, a triangular bone at the base of the spine that connects to the pelvis.
A common external landmark for approximating the location of L4 and L5 is the top of the iliac crests, which are the prominent bones at the top of your hips. Drawing an imaginary line across the highest points of these crests typically intersects the body of the L4 vertebra or the intervertebral disc space between L4 and L5.
The Role of L4 and L5
The L4 and L5 vertebrae, along with the intervertebral disc nestled between them, form a functional spinal segment. One of their primary roles involves bearing a substantial portion of the body’s upper weight. As the lowest segments of the lumbar spine, they act as a foundational support system, transferring loads from the upper body to the pelvis and lower limbs.
Beyond weight bearing, this spinal segment facilitates a range of movements in the lower back. These movements include flexion (bending forward), extension (arching backward), and rotation (twisting the torso). The L4 and L5 vertebrae are instrumental in protecting delicate nerve roots that exit the spinal canal at these levels. The L4 and L5 nerve roots emerge from the spinal cord through openings called foramina, located between the respective vertebrae. These nerve roots contribute to the formation of larger nerves, including portions of the sciatic nerve, which are essential for sensation and motor function in the legs and feet.
Common Conditions Affecting L4 and L5
Due to their position at the base of the mobile lumbar spine and their role in weight-bearing and movement, the L4 and L5 spinal segments are frequently susceptible to various conditions. The cumulative stress from daily activities and the transition zone between the mobile lumbar spine and the more rigid sacrum contribute to their vulnerability.
One prevalent condition is disc herniation, often referred to as a “slipped disc,” where the soft inner material of the intervertebral disc pushes through its tougher outer layer. This can compress the nearby L4 or L5 nerve roots, resulting in symptoms such as lower back pain that may radiate down the leg, numbness, or weakness in specific muscle groups. The L4-L5 disc is one of the most common sites for such occurrences.
Spinal stenosis is another condition that can affect the L4 and L5 levels, characterized by a narrowing of the spinal canal or the nerve root openings. This narrowing can put pressure on the spinal cord or exiting nerve roots, leading to symptoms like pain, tingling, or weakness in the legs, often exacerbated by standing or walking. Degenerative changes in the vertebrae and discs are common causes of this narrowing.
Spondylolisthesis, which involves the slipping of one vertebra forward over another, frequently occurs at the L5-S1 level, but can also involve L4 slipping over L5. This displacement can compress nerve roots or cause mechanical instability, leading to lower back pain, muscle spasms, or neurological symptoms in the legs.
Degenerative disc disease, a general term for age-related wear and tear on the intervertebral discs, often impacts the L4 and L5 discs, causing them to lose height and elasticity. This can lead to chronic low back pain, stiffness, and increased susceptibility to other spinal issues due to reduced shock absorption.
