His work provided empirical evidence that something unseen was being transferred from patient to patient. Joseph Lister built upon this foundation in the 1860s by developing antiseptic surgical techniques, utilizing carbolic acid to disinfect instruments and wounds. These advancements established the theoretical basis for infection control, yet they focused on chemical disinfection, which was harsh on the skin, rather than a physical barrier for the hands.
The Introduction of Surgical Rubber Gloves
The first practical use of rubber gloves in a medical setting occurred in the late 1880s at Johns Hopkins Hospital, driven by an occupational health concern. Surgeon William Halsted commissioned the Goodyear Rubber Company to create thin rubber gloves for his scrub nurse, Caroline Hampton. Hampton had developed severe contact dermatitis from repeatedly plunging her hands into the strong antiseptic solutions used for surgical scrubbing.
Halsted’s initial motivation was solely to protect his nurse’s hands from the irritating chemicals, not to prevent patient infection. The custom-made rubber gloves proved successful in protecting her skin, and other assistants soon began wearing them for the same reason. The high virulence and the fear surrounding transmission forced a dramatic reevaluation of infection control protocols across all healthcare fields.
In 1985, the Centers for Disease Control and Prevention (CDC) introduced the concept of “Universal Precautions” in response to the epidemic. This standard dictated that all patients should be treated as if they are potentially infectious, regardless of their known health status. The new guidelines explicitly mandated the use of barrier protection, including gloves, masks, and eyewear, for all dental procedures that involved contact with blood, saliva, or mucous membranes.
