What Is the Difference Between LPN and RN?

Licensed practical nurses (LPNs) and registered nurses (RNs) both provide patient care, but they differ significantly in education, clinical authority, work settings, and pay. RNs earn a median salary of $93,600 per year compared to $62,340 for LPNs, and they carry broader responsibility for patient assessment, care planning, and supervision. Understanding these differences matters whether you’re choosing a nursing path or trying to understand the roles of the people caring for you.

Education and Training

LPN programs are typically certificate or diploma programs that take about 12 months to complete. They cover foundational nursing skills, basic pharmacology, and patient care techniques. You can find these programs at community colleges and vocational schools, and they’re designed to get you into the workforce quickly.

RNs have two main educational paths. An Associate Degree in Nursing (ADN) usually takes two to three years. A Bachelor of Science in Nursing (BSN) takes four years and includes more coursework in leadership, research, public health, and critical thinking. Many hospitals now prefer or require a BSN for new hires, so the trend has been moving toward the four-year degree even though an ADN qualifies you for the same licensing exam.

Licensing Exams

Both roles require passing a version of the NCLEX (National Council Licensure Examination), but the two tests are different. The NCLEX-PN for LPNs has 85 to 205 questions and a five-hour time limit. It focuses on care coordination, data collection, basic comfort measures, and safety protocols.

The NCLEX-RN has 75 to 265 questions with a six-hour time limit. It places heavier emphasis on physical sciences, clinical assessment, management of care, and ethical and legal decision-making. It also tests knowledge of advanced therapies like IV infusions, blood transfusions, and central venous access devices. Both exams share the same four broad content areas, but the RN version goes deeper into independent clinical judgment.

Scope of Practice

This is where the two roles diverge most sharply. RNs are responsible for the initial comprehensive assessment of a patient, the formulation of nursing diagnoses, and the development of care plans. They interpret clinical data independently and coordinate across care teams to adjust treatment as a patient’s condition changes. The American Nurses Association describes RN practice as holistic, taking into account physical, psychosocial, and spiritual needs.

LPNs cannot independently perform comprehensive assessments, formulate nursing diagnoses, or create care plans. Instead, they carry out nursing interventions under RN supervision. Their day-to-day work includes monitoring vital signs, updating health records, administering medications, assisting with basic procedures, and reporting changes in patient status to the supervising RN or physician. Certain clinical tasks, including specific components of IV therapy, fall outside the LPN scope of practice in most states.

State nursing boards set the exact boundaries, and they vary. But the general principle is consistent: the RN leads and the LPN supports. In North Carolina, for example, an LPN’s practice must be clinically supervised by an RN at all times. An LPN cannot supervise an RN or serve as a nurse manager, head nurse, or charge nurse.

Where Each Role Works

RNs and LPNs tend to work in very different settings. Over 59% of the 3.4 million RNs in the U.S. work in hospitals. Only 6% work in nursing and residential care facilities. Hospitals need the advanced assessment skills, care planning authority, and clinical judgment that RN licensure provides.

LPNs concentrate in long-term and outpatient care. About 37% of the roughly 651,400 LPNs work in nursing and residential care facilities, where patients need consistent daily support over extended periods. Only 16% of LPNs work in hospitals. Another 12% work in home healthcare services, and 12% in physicians’ offices. These environments tend to involve more routine, predictable care where the LPN skill set aligns well with patient needs.

Salary and Compensation

The pay gap between the two roles reflects the difference in education and responsibility. As of May 2024, the Bureau of Labor Statistics reports a median annual wage of $93,600 for RNs and $62,340 for LPNs (about $29.97 per hour). That’s a difference of more than $31,000 per year at the median.

Compensation varies by state, specialty, and employer. RNs who work in hospitals or specialized units generally earn more than those in outpatient settings. LPNs in metropolitan areas or states with higher costs of living also tend to earn above the national median. Overtime, shift differentials for nights and weekends, and employer benefits packages all factor into total compensation for both roles.

Clinical Supervision and Accountability

The legal relationship between the two roles is hierarchical. RNs are responsible for the ongoing supervision, teaching, and evaluation of nursing personnel, including LPNs. This means the RN must be continuously available for direct participation in care, must assess whether the LPN’s capabilities match the patient’s needs, and is ultimately accountable for the nursing care delivered by the staff they oversee.

For LPNs, this supervision requirement is a defining feature of the role. You practice under the direction of an RN (or, in some states, a physician or other authorized provider). You carry out assigned interventions, collect and report data, and flag concerns. But the responsibility for interpreting that data and adjusting the care plan stays with the RN.

Moving From LPN to RN

If you’re already working as an LPN and want to advance, LPN-to-RN bridge programs offer a streamlined path. These programs grant advanced standing so you can skip the first year of a standard RN program. At Johnson County Community College, for example, bridge students complete three semesters of nursing coursework plus general education requirements, then sit for the NCLEX-RN. Completing two transitional courses earns automatic credit for the foundational nursing courses that first-year students take.

Bridge programs are competitive, and you’ll need to finish prerequisite coursework before enrolling. But the overall timeline is significantly shorter than starting an RN program from scratch, and you enter with clinical experience that gives you a practical advantage. Many LPNs continue working part-time while completing bridge programs, though the course load is demanding.

Choosing Between the Two Paths

The LPN route makes sense if you want to enter healthcare quickly with a shorter, less expensive educational commitment. It gives you hands-on patient care experience within about a year, and it can serve as a stepping stone to an RN degree later. LPN roles are especially abundant in long-term care, home health, and outpatient clinics.

The RN path requires a bigger upfront investment in education but opens the door to higher pay, a wider range of work settings (especially hospitals), greater clinical autonomy, and leadership roles. RNs can also specialize in areas like critical care, oncology, pediatrics, or surgery, and can pursue advanced practice roles like nurse practitioner with further graduate education.
