High arches, known medically as pes cavus, present as an unusually elevated arch that does not flatten even when weight is applied. When a footprint is made, there is little to no connection between the heel and forefoot areas. The elevated arch can lead to excessive pressure on the ball and heel of the foot during standing or walking.
High arches are associated with underlying neurological conditions such as cerebral palsy, Charcot-Marie-Tooth disease, spina bifida, polio, muscular dystrophy, or stroke. In some cases, a high arch can be an inherited structural variation. Symptoms include pain in the foot, especially in the ball or heel, and ankle pain or swelling. Individuals also experience instability, leading to frequent ankle sprains, or develop calluses on the ball, side, or heel of the foot due to increased pressure. Bent or claw toes are common deformities.
When Arches Vary: Implications and General Approaches
Variations in foot arch, whether flat or high, can influence gait, balance, and footwear choices, though not all variations cause problems. Flat feet can lead to overpronation, where the foot rolls excessively inward during walking or running, increasing stress on the knees, hips, and lower back. This altered alignment can also contribute to issues like shin splints or plantar fasciitis. Conversely, high arches result in underpronation, or supination, where the foot rolls outward, placing more strain on the outer edge of the foot and lower leg. This can reduce the foot’s natural shock absorption, leading to conditions like metatarsalgia or increased susceptibility to ankle sprains.
Managing comfort and issues related to arch variations involves choosing appropriate footwear. Individuals with flat feet benefit from shoes with built-in arch support and adequate cushioning to maintain proper foot alignment and reduce excessive pronation. For those with high arches, cushioned footwear is recommended to absorb impact and alleviate pressure on the heel and ball of the foot. Over-the-counter arch supports or custom orthotic inserts can provide additional support and distribute weight more evenly.
