Wegovy can be injected in three places: the abdomen, the front of the thigh, or the upper arm. None of these sites is clinically superior to the others. FDA pharmacokinetic reviews found that injection site was of “no or minor importance” for how much semaglutide your body absorbs, so the best spot is whichever one feels most comfortable and is easiest for you to reach.
The Three Approved Injection Sites
Wegovy is a subcutaneous injection, meaning the needle goes into the fatty layer just beneath the skin, never into muscle or a vein. The pre-filled pen has a short, fine needle designed for exactly this depth. Each of the three sites has a slightly different feel.
- Abdomen: The lower stomach area, at least two inches away from your belly button in any direction. Most people find this site easiest to access and pinch. The abdomen generally has a reliable layer of subcutaneous fat, which is why many healthcare providers suggest starting here.
- Front of the thigh: The middle third of the front of your upper leg. This is a good option if you prefer injecting while seated and want to see what you’re doing. Some people report slightly more sensation here than in the abdomen because the thigh can have less subcutaneous fat.
- Upper arm: The outer, fleshy area of the upper arm. This site works well but can be harder to reach on your own. If someone else is giving you the injection, the upper arm is a convenient choice.
Why Absorption Doesn’t Favor One Site
With some injectable medications, the abdomen absorbs the drug faster than other sites. That’s not a meaningful concern with Wegovy. The FDA’s clinical pharmacology review compared semaglutide exposure across injection sites and found all three fell within the standard bioequivalence range. In practical terms, you get the same amount of medication in your bloodstream regardless of where you inject. This is partly because semaglutide is a long-acting drug dosed once per week, so small differences in absorption speed wash out over the course of a week.
How to Rotate Sites Each Week
Rotating your injection site helps prevent skin irritation and a condition called lipohypertrophy, where the fatty tissue under the skin thickens or hardens from repeated needle use in one spot. A simple approach is to cycle through all three areas: abdomen one week, thigh the next, upper arm the third week, then back to the abdomen.
You can also rotate within a single area. If you prefer the abdomen, for example, inject on the left side one week and the right side the next, or move a couple of inches from where you injected previously. The key is to avoid hitting the exact same spot week after week.
Injection Site Reactions
Local reactions at the injection site are uncommon with Wegovy. In clinical trials, only 1.4% of people using Wegovy reported any injection site reaction, compared to 1% on placebo. When reactions did occur, they included mild redness, itching, swelling, or irritation. These typically resolve on their own within a day or two.
If you notice a spot that stays red, feels hard, or becomes increasingly painful after several days, that’s worth mentioning to your provider. But for most people, injection site discomfort is minimal and short-lived.
Tips for a Comfortable Injection
Let the pen sit at room temperature for about 30 minutes before injecting. A cold pen straight from the refrigerator can sting more going in. Wegovy pens are stored in the refrigerator at 36°F to 46°F, but they can safely stay at room temperature (up to 86°F) for up to 28 days if needed.
When you’re ready to inject, pinch the skin at your chosen site to lift the fatty layer away from the muscle underneath. Press the pen firmly against the skin and hold it in place until the pen clicks and the dose window shows the injection is complete. Don’t rush this step. Pulling the pen away too early can mean you don’t get the full dose.
Avoid injecting into areas where the skin is bruised, tender, scarred, or has stretch marks. These areas can affect how comfortably the needle enters and how evenly the medication disperses. Choose a spot where the skin is smooth and you can easily pinch a fold of tissue.
