What Is MUSE Alprostadil and How Is It Used?

MUSE Alprostadil is a prescription medication used to treat erectile dysfunction (ED) in adult males. The acronym MUSE stands for Medicated Urethral System for Erections. It offers an alternative for individuals who may not be candidates for or responsive to oral medications. This treatment involves the direct application of medication to achieve an erection.

Understanding How MUSE Works

MUSE Alprostadil contains the active ingredient alprostadil, a synthetic version of prostaglandin E1 (PGE1), a naturally occurring substance in the body. Prostaglandin E1 acts as a vasodilator, meaning it causes blood vessels to widen. When alprostadil is administered, it is absorbed into the corpus spongiosum, a spongy tissue surrounding the urethra.

From the corpus spongiosum, a portion of the alprostadil is transported to the corpora cavernosa, the two main chambers in the penis that fill with blood during an erection. The medication works by relaxing the smooth muscles within the penile arteries and tissues. This relaxation allows for increased blood flow into the penis, which helps to produce and maintain an erection.

Administering MUSE Alprostadil

MUSE Alprostadil is a single-use transurethral system, delivering a small medicated pellet directly into the male urethra. Before administration, it is advisable to urinate, as residual urine helps disperse the medicated pellet and facilitates absorption. The applicator stem is then inserted into the urethra, about an inch deep.

To release the pellet, a button on the applicator is depressed. After insertion, gently massaging or “kneading” the penis for approximately 10 to 30 seconds helps distribute the medication evenly and promotes absorption. Remaining in a sitting, standing, or walking position for the first 10 minutes after use can further aid absorption.

The usual starting dose of alprostadil is 250 micrograms (mcg), though doses can range up to 1000 mcg, as determined by a healthcare provider based on individual response. The medication should not be used more than twice within a 24-hour period. Each applicator is for single use only and must be discarded safely.

Essential Information About MUSE

MUSE Alprostadil begins to work within 5 to 10 minutes after administration. The effects last for approximately 30 to 60 minutes, though this duration can vary among individuals. The medication is rapidly absorbed, with about 80% of an administered dose absorbed within 10 minutes, and it does not remain in the system for an extended period.

Common side effects include localized reactions such as pain or burning in the penis or urethra. Mild bleeding or spotting from the urethra, or pain in the testes may also occur. Other side effects include flu-like symptoms, general body aches, or a runny nose.

More serious side effects include prolonged erections (priapism) lasting four hours or more, which requires immediate medical attention to prevent permanent damage. Dizziness or fainting, particularly within an hour of administration, can also occur due to a lowering of blood pressure. Individuals should avoid driving or operating heavy machinery if they experience these effects.

MUSE may not be suitable for everyone. It is contraindicated in individuals with certain penile conditions, such as severe curvature or birth defects, or a history of priapism. It should also be used with caution or avoided in individuals with bleeding problems or heart conditions. If engaging in sexual activity with a pregnant partner, a condom barrier is recommended, as alprostadil can be present in ejaculate and may induce uterine contractions.
