Degenerative Joint Disease (DJD) is the medical term most commonly used to describe Osteoarthritis (OA), a widespread condition affecting millions of people globally. This chronic disease involves the progressive deterioration of joints, primarily due to mechanical stress and biological processes. Often characterized as a “wear and tear” condition, DJD causes pain, stiffness, and reduced mobility that typically worsens over time. It represents the most prevalent form of arthritis, and while the damage cannot be reversed, various management strategies can significantly help control symptoms and maintain function.

Defining Degenerative Joint Disease and Its Mechanism

DJD is a progressive joint disorder that involves the entire joint structure, though its defining feature is the breakdown of articular cartilage. Articular cartilage is the smooth, protective tissue that covers the ends of bones where they meet to form a joint, acting as a shock absorber and allowing for frictionless movement. In DJD, this cartilage softens, frays, and eventually wears away, decreasing the space between the bones.

As the protective cartilage deteriorates, the underlying bone is exposed, leading to direct bone-on-bone contact, which causes pain and further damage. The body attempts to repair this damage by forming new bone outgrowths, known as osteophytes or bone spurs, around the joint margins. These osteophytes alter the joint’s normal mechanics and contribute to stiffness and reduced range of motion. Obesity is another major modifiable risk factor because excess body weight places increased mechanical load and stress on weight-bearing joints like the knees and hips, accelerating cartilage breakdown. Genetic factors also play a role, as a family history of osteoarthritis can increase an individual’s susceptibility to the condition. Furthermore, occupational stress from jobs requiring repetitive motions or heavy lifting can contribute to the development of DJD in the overstressed joints.

Managing DJD focuses on controlling pain, improving function, and slowing the progression of the disease through a combination of non-surgical methods. Weight management is a cornerstone strategy, particularly for affected weight-bearing joints, since losing a single pound of body weight can reduce the pressure on the knees by several pounds.

Appropriate low-impact exercise, such as swimming, cycling, or walking, is highly recommended to maintain joint mobility and strengthen the muscles that support the joint without causing excessive stress. Physical therapy is often prescribed to teach specific exercises that improve range of motion and muscle strength around the affected joint, helping to stabilize it.

Pain relief can be achieved through over-the-counter medications, such as acetaminophen or nonsteroidal anti-inflammatory drugs (NSAIDs), though these should be used under the guidance of a physician.
