What Is Blisovi Fe? Uses, Side Effects, and More

Blisovi Fe is a combination birth control pill that contains two hormones, norethindrone acetate and ethinyl estradiol, plus a small amount of iron in its inactive pills. It comes in a 28-day pack and is taken once daily to prevent pregnancy. The “Fe” in the name stands for iron (ferrous fumarate), which is included in the placebo pills you take during the last four days of each pack.

How the Pack Is Set Up

Each Blisovi Fe pack contains 28 tablets taken in a specific order. The first 24 are white active pills, each containing 1 mg of norethindrone acetate and 20 mcg of ethinyl estradiol. The last 4 are brown placebo pills that contain 75 mg of ferrous fumarate (an iron supplement) but no hormones.

You take one pill at the same time every day for all 28 days. The brown pills exist mainly to keep you in the habit of taking a daily pill so you don’t forget to start your next pack on time. Your period typically arrives during those four placebo days. The iron in them doesn’t serve a direct therapeutic purpose for contraception, but it provides a small supplemental dose of iron during the days when you’re menstruating and losing some blood.

There is also a version called Blisovi Fe 1.5/30, which has a slightly higher dose of both hormones. The version described above, Blisovi 24 Fe (sometimes labeled Blisovi Fe 1/20), is the lower-dose formulation and the more commonly discussed one.

How It Prevents Pregnancy

Blisovi Fe works through two main mechanisms. The primary one is stopping ovulation entirely. The progestin component (norethindrone acetate) sends signals to the brain that slow the release of the hormones responsible for developing and releasing an egg each month. Without a mature egg, pregnancy can’t occur.

The second mechanism involves changes to cervical mucus. The progestin thickens the mucus at the opening of the cervix, making it much harder for sperm to travel through and reach the upper reproductive tract. These two effects working together make combination pills like Blisovi Fe highly effective when taken consistently.

Common Side Effects

In a study of 743 women taking Blisovi 24 Fe, the most frequently reported side effects were:

  • Headache: 6.3%
  • Vaginal yeast infections: 6.1%
  • Nausea: 4.6%
  • Menstrual cramps: 4.4%
  • Breast tenderness: 3.4%
  • Mood changes (mood swings or depression): about 3.3% combined
  • Bacterial vaginitis: 3.1%
  • Acne: 2.7%
  • Weight gain: 2.0%

Other side effects reported with combination birth control pills in general include breakthrough bleeding or spotting (especially in the first few months), bloating, changes in menstrual flow, and changes in weight in either direction. Some women also notice contact lens discomfort due to subtle changes in the shape of the cornea. Most of these side effects are mild and often improve after the first two to three cycles as your body adjusts.

Smoking and Cardiovascular Risk

All combination birth control pills, including Blisovi Fe, carry a serious warning about cigarette smoking. Smoking while taking estrogen-containing pills significantly increases the risk of blood clots, stroke, and heart attack. This risk climbs sharply for women over 35 who smoke. If you smoke and are over 35, combination pills like Blisovi Fe are not considered safe, and a progestin-only method or non-hormonal option is a better fit.

Even for non-smokers, combination pills carry a small baseline increase in the risk of blood clots compared to not using hormonal contraception. This risk is still quite low for most healthy women but is worth understanding, particularly if you have a personal or family history of clotting disorders, migraines with aura, or certain heart conditions.

What to Do If You Miss a Pill

Timing matters with Blisovi Fe. Taking your pill at the same time each day keeps hormone levels steady and maximizes effectiveness. If you miss one active (white) pill, take it as soon as you remember, even if that means taking two pills in one day. If you miss two or more active pills, your risk of pregnancy increases, and you may need to use a backup method like condoms for the next seven days. Check the instructions in your specific pack or contact your prescriber, because the exact guidance depends on which week of the pack you’re in when the miss happens.

If you miss any of the brown placebo pills, it doesn’t affect your pregnancy protection at all. Those pills contain no hormones. Just discard the missed brown pill and continue with your pack as usual, making sure to start your next pack of active pills on schedule.

How Blisovi Fe Compares to Other Pills

Blisovi Fe is a generic equivalent of the brand-name pill Loestrin Fe. It contains the same active ingredients at the same doses, so it works the same way and has the same side effect profile. You may also see it compared to Junel Fe, Larin Fe, and Microgestin Fe, all of which are generics in the same family. The differences between these are primarily in manufacturer, pill coating, and inactive ingredients, not in the hormones themselves. If you’ve been switched from one of these to Blisovi Fe (or vice versa) by your pharmacy, the contraceptive effect is the same.

The 24-day active pill format (24 active, 4 placebo) gives you a shorter hormone-free window than the older 21/7 format used by some pills. This shorter gap can mean lighter, shorter periods for some women and may provide slightly more reliable ovulation suppression, since there’s less time for your body to “wake up” hormonally before the next pack begins.
