What Is an LPN? Roles, Requirements, and Salary

An LPN, or licensed practical nurse, is a healthcare professional who provides basic nursing care under the supervision of registered nurses and physicians. LPNs are the front line of hands-on patient care in nursing homes, hospitals, clinics, and private homes, handling everything from checking vital signs to changing bandages to helping patients with daily tasks like bathing and eating. In some states, the same role is called a licensed vocational nurse, or LVN.

What LPNs Do Day to Day

The core of an LPN’s work is monitoring patients and keeping them comfortable. That means checking blood pressure, temperature, and other vital signs on a regular schedule, then documenting everything in the patient’s health record. LPNs change wound dressings, assist with bathing and dressing, and talk with patients about their care and concerns. When something changes or a patient raises an issue, the LPN reports it to a registered nurse or physician.

The specifics shift depending on where the LPN works. In a nursing home, an LPN might help residents eat meals and manage their daily medications. In a hospital, the work leans more toward collecting lab samples and running routine tests. In home health care, an LPN often reinforces care instructions for family members who are looking after a loved one. In some states, LPNs are also authorized to administer medications or start IV drips, though this varies by state law.

How LPNs Differ From Registered Nurses

The biggest distinction between an LPN and a registered nurse (RN) comes down to independence. RNs perform comprehensive patient assessments, formulate nursing diagnoses, and build the overall care plan. LPNs work in a dependent role: they collect patient data and flag concerns, but they don’t independently interpret clinical findings or develop the care plan themselves. Instead, they collaborate with RNs and help carry out the plan that’s already been established.

Think of it this way. If a patient’s condition changes overnight, the LPN recognizes the change and determines whether immediate intervention is needed, but the RN is the one who reassesses the full clinical picture, updates the diagnosis, and adjusts the care strategy. LPNs execute medical orders prescribed by physicians, nurse practitioners, physician assistants, dentists, midwives, or podiatrists. They are not permitted to carry out orders from pharmacists, nutritionists, or other non-prescribing professionals.

This supervision structure doesn’t mean LPNs lack skill or judgment. They’re trained to notice problems early and act quickly within their scope, which makes them essential in settings like long-term care facilities where they often spend more face time with patients than anyone else on the care team.

Education and Licensing Requirements

Becoming an LPN is one of the faster paths into nursing. Training programs, typically offered through community colleges and technical schools, run about 12 months. The prerequisites are straightforward: a high school diploma or GED, and in some cases a background check or basic skills test before enrollment.

Coursework covers fundamentals like anatomy, pharmacology, patient care techniques, and clinical rotations where students practice skills in real healthcare settings. After completing the program, graduates must pass the NCLEX-PN, the national licensing exam for practical nurses. If you don’t pass on the first attempt, you can retake it after a 45-day waiting period. Once licensed, you can begin working immediately.

Where LPNs Work

Nursing and residential care facilities employ a large share of LPNs, which makes sense given how much direct patient care these settings require around the clock. Hospitals, physicians’ offices, and home health agencies are also common employers. Government facilities, including Veterans Affairs hospitals and public health clinics, hire LPNs as well.

The work environment shapes the pace and intensity of the job. A physician’s office tends to follow a predictable daytime schedule, while hospitals and nursing homes often require evening, night, and weekend shifts. Home health positions may involve traveling between patients’ residences throughout the day.

Salary and Job Demand

As of May 2023, the median annual wage for LPNs was $59,730, which works out to about $28.72 per hour. Pay varies by location and setting. LPNs in metropolitan areas and hospital systems generally earn more than those in rural clinics or smaller facilities. Demand for LPNs remains steady, driven largely by an aging population that needs more long-term and residential care.

Moving From LPN to RN

Many LPNs use the role as a stepping stone toward becoming a registered nurse. LPN-to-RN bridge programs offer advanced placement, meaning you get credit for the clinical knowledge you already have rather than starting a nursing degree from scratch. These programs typically award an associate degree in nursing and total around 70 credits, though some of those credits are granted based on your existing LPN training and experience.

Prerequisites for bridge programs generally include courses in anatomy and physiology, microbiology, psychology, sociology, college algebra, and English composition. You’ll also need an active LPN license and a minimum GPA, often around 2.5. The timeline varies, but most LPNs can complete a bridge program in one to two years while continuing to work part-time, making it a practical route to expanded responsibilities and higher pay.
