An advanced practice nurse, formally called an Advanced Practice Registered Nurse (APRN), is a registered nurse who has completed graduate-level education and national certification in one of four specialized roles. APRNs go beyond the scope of a standard RN: they can diagnose conditions, order and interpret tests, prescribe medications, and manage treatment plans. There are roughly four distinct career paths within advanced practice nursing, each with its own focus and patient population.
The Four APRN Roles
The regulatory framework that defines advanced practice nursing recognizes exactly four roles: certified nurse practitioner (CNP), certified registered nurse anesthetist (CRNA), certified nurse-midwife (CNM), and clinical nurse specialist (CNS). This framework comes from the Consensus Model for APRN Regulation, developed in 2008 by a joint effort between the Advanced Practice Nursing Consensus Work Group and the National Council of State Boards of Nursing. Every APRN in the United States falls into one of these four categories.
Nurse Practitioners
Nurse practitioners are the most widely recognized type of APRN. Much like physicians, they examine patients, diagnose illnesses, interpret lab results and imaging, prescribe medications, and provide health counseling. In practice, NPs record health histories, perform complete physical exams, diagnose and treat common acute and chronic problems, manage medications and therapies, and refer patients to specialists when needed. Many NPs serve as primary care providers, particularly in underserved or rural areas where physician shortages are common. They work across specialties including family medicine, pediatrics, psychiatry, and geriatrics.
Certified Registered Nurse Anesthetists
CRNAs deliver anesthesia, pain management, and related care in operating rooms, dental offices, labor and delivery units, and other clinical settings. Research has consistently shown that safe anesthesia services can be provided by CRNAs, anesthesiologists, or teams of both professionals working together. During the early phases of the COVID-19 pandemic, 13 states relaxed supervision requirements for CRNAs, allowing them to practice without physician oversight. In many of those states, CRNAs were fully utilizing their training and providing expertise well beyond the operating room.
Certified Nurse-Midwives
CNMs are independent practitioners who provide the full spectrum of women’s health care. That includes prenatal care, labor and delivery, postpartum care, well-woman visits, contraception, management of common gynecological problems, and menopause care. Their scope extends well beyond delivering babies, though that remains the most visible part of the role.
Clinical Nurse Specialists
Clinical nurse specialists operate across three spheres of impact: direct patient care, nursing practice, and organizational systems. This makes the CNS role unique among APRNs. A CNS might spend part of their time caring for patients directly, part coaching and mentoring other nurses, and part redesigning hospital protocols to improve outcomes. They often specialize in areas like oncology, critical care, or mental health.
Education Requirements
Every APRN must complete an accredited graduate-level education program. The minimum has traditionally been a Master of Science in Nursing (MSN), but the field is shifting toward doctoral preparation. The Doctor of Nursing Practice (DNP) builds on master’s-level content by adding training in evidence-based practice, quality improvement, and systems leadership. It’s designed as a terminal practice degree, distinct from research-focused doctoral programs like a PhD.
The National Organization of Nurse Practitioner Faculties called for the DNP to become the entry-level degree for nurse practitioners by 2025 and reaffirmed that position in April 2023. For nurse anesthetists, the transition is already complete: as of January 2022, every student entering an accredited CRNA program enrolls in a doctoral program. In 2024, post-baccalaureate DNP programs were offered at 307 schools, post-master’s DNP programs at 403 schools, and entry-level (prelicensure) DNP programs at 6 schools.
Certification and Licensure
Earning a degree isn’t enough on its own. APRNs must also pass a national certification exam that measures competencies specific to their role and patient population, then maintain that certification through ongoing recertification. The American Nurses Credentialing Center (ANCC), which has set credentialing standards since 1990, is one of the primary certifying bodies. Other organizations certify in specific roles, such as nurse anesthesia or midwifery.
The regulatory framework for APRNs rests on four pillars, known by the acronym LACE: licensure (the legal authority to practice), accreditation (formal approval of educational programs), certification (demonstrated mastery of role-specific standards), and education (completion of a qualifying graduate program). All four must be in place before an APRN can practice independently.
Prescribing Authority and Practice Autonomy
APRNs can prescribe medications, including controlled substances, but the specifics vary dramatically by state. The federal Drug Enforcement Administration allows nurse practitioners, nurse-midwives, nurse anesthetists, and clinical nurse specialists to register for DEA numbers and prescribe controlled substances, provided their state authorizes it.
State practice laws fall into three categories. Full practice states allow NPs to evaluate patients, diagnose, order tests, and prescribe medications (including controlled substances) under the sole authority of the state board of nursing, with no requirement for physician involvement. This is the model recommended by both the National Academy of Medicine and the National Council of State Boards of Nursing. Reduced practice states require NPs to maintain a career-long collaborative agreement with another health provider, or they limit at least one element of practice. Restricted practice states go further, requiring ongoing supervision, delegation, or team management by another provider.
These distinctions matter most to nurse practitioners. CRNAs face their own state-by-state patchwork of supervision requirements, while CNMs and CNSs navigate similar but separate regulatory landscapes.
What Sets APRNs Apart From RNs
The formal definition spells it out: APRN practice builds on the competencies of registered nurses by demonstrating greater depth and breadth of knowledge, more complex skills and interventions, increased ability to synthesize clinical data, and greater autonomy. Where an RN might carry out a treatment plan, an APRN creates one. APRNs are educationally prepared to take responsibility for health promotion, assessment, diagnosis, and management of patient problems using both medication and non-medication approaches.
A significant component of APRN education and practice centers on direct care of individual patients. This is the defining factor that separates APRNs from nurses in administrative or purely educational roles, even those with graduate degrees.
Salary by Role
Compensation reflects the specialized training and responsibility these roles carry. According to Bureau of Labor Statistics data from May 2024, the median annual wages break down as follows:
- Nurse anesthetists: $223,210
- Nurse practitioners: $129,210
- Nurse midwives: $128,790
The substantial gap between CRNAs and the other roles reflects the intensity of anesthesia training (now exclusively at the doctoral level) and the high-stakes nature of the work. Nurse practitioners and nurse-midwives earn comparable salaries, though both can vary widely depending on specialty, geographic location, and practice setting.
