Ambulatory nursing is a specialty focused on caring for patients who are not admitted to a hospital. These nurses work in outpatient settings like clinics, physician offices, surgery centers, and telehealth programs, providing care that patients receive and go home the same day. It’s one of the fastest-growing areas in nursing, driven by a healthcare system that increasingly moves services out of hospital beds and into community-based settings.
Where Ambulatory Nurses Work
The defining feature of ambulatory nursing is the setting: anywhere patients receive care without an overnight hospital stay. The American Academy of Ambulatory Care Nursing (AAACN) defines it as a specialty that provides care “across the lifespan to individuals, families, caregivers, groups, communities, and populations in primary, specialty, and non-acute outpatient settings.”
In practice, that covers a wide range of workplaces. Primary care offices and family medicine clinics are among the most common. Specialty clinics for cardiology, oncology, endocrinology, or orthopedics employ ambulatory nurses to manage complex patient populations. Ambulatory surgery centers, urgent care facilities, community health centers, employer-based health clinics, and Federally Qualified Health Centers (FQHCs) all fall under this umbrella. Even call centers that handle patient triage calls are considered ambulatory care settings.
What Ambulatory Nurses Actually Do
The day-to-day work looks different from bedside hospital nursing. Rather than monitoring patients continuously over a shift, ambulatory nurses typically see a high volume of patients for shorter interactions. Their core responsibilities center on a few key areas.
Triage and assessment: One of the most critical skills. Ambulatory nurses evaluate patients who call or walk in, determine the urgency of their symptoms, and decide whether they need an immediate appointment, emergency care, or home management advice. Phone and electronic triage make up a significant portion of the workload in many settings.
Patient education: Because patients leave the same day, ambulatory nurses spend considerable time teaching people how to manage conditions at home. This includes explaining medications, demonstrating wound care, walking patients through post-procedure instructions, and coaching people with chronic diseases like diabetes or heart failure on lifestyle changes.
Care coordination: Ambulatory nurses often serve as the connective tissue between a patient and the broader healthcare system. They schedule referrals, follow up on lab results, coordinate between specialists, and ensure patients don’t fall through the cracks between appointments. This role is especially important for patients managing multiple chronic conditions who see several providers.
Chronic disease management: Many ambulatory nurses work with patients over months or years, tracking their progress with conditions like hypertension, diabetes, or asthma. They monitor trends in lab work, adjust care plans in collaboration with physicians, and intervene early when a patient’s condition starts to worsen.
The Growing Role of Telehealth
Telehealth has become deeply woven into ambulatory nursing. Nurses in outpatient settings use video visits and remote monitoring tools to communicate with patients, provide counseling and coaching, deliver education, and oversee ongoing care management. For chronic conditions like heart disease and diabetes, nurses review biometric data that patients submit from home, such as blood pressure readings, glucose levels, or weight, then coordinate with the patient’s provider and conduct follow-up video sessions.
This shift is accelerating. Projections from the American Hospital Association estimate that by 2034, nearly one in four evaluation and management visits will be delivered virtually, with half of all psychotherapy visits happening through telehealth. Outpatient volumes overall are projected to jump 17% over the coming decade, and home health is expected to grow 22%. For ambulatory nurses, this means telehealth competency is quickly becoming a baseline expectation rather than a bonus skill.
In Federally Qualified Health Centers, nurses have taken on leadership roles in telehealth, developing protocols for reimbursement, staff training, and safe patient connections. The ambulatory nurse in these settings isn’t just delivering care through a screen; they’re building the systems that make virtual care possible.
How It Differs From Hospital Nursing
The biggest difference is the patient relationship. Hospital nurses care for acutely ill patients over hours or days, then hand off to the next shift. Ambulatory nurses often build long-term relationships, seeing the same patients repeatedly over months or years. You get to watch patients improve, which many nurses find deeply rewarding.
The pace is different, too. Instead of monitoring a handful of patients intensively, ambulatory nurses may interact with dozens of patients in a single day through a mix of in-person visits, phone calls, messages, and video appointments. The clinical acuity is generally lower, but the volume and variety are higher. You need sharp assessment skills because you’re making quick decisions with limited information, often over the phone, about whether a patient’s symptoms can wait or need urgent attention.
Work schedules tend to follow business hours more closely than hospital nursing. Evenings, nights, and weekends are less common (though not unheard of, especially in urgent care). This predictability is a significant draw for nurses seeking better work-life balance.
Certification and Career Path
Ambulatory nursing doesn’t require a special certification to practice, but earning one signals expertise and can open doors. The American Nurses Credentialing Center offers the Ambulatory Care Nurse, Board Certified (AMB-BC) credential. To qualify, you need an active RN license, at least two years of full-time nursing experience, a minimum of 2,000 hours of clinical practice specifically in ambulatory care within the past three years, and 30 hours of continuing education in the specialty within the same period.
The certification exam itself is a three-hour test with 150 questions, 125 of which are scored. It covers the nursing process, care coordination, patient education, and professional performance standards. The AAACN defines two main domains in the specialty’s scope of practice: professional clinical practice and nursing organizational and professional performance, spanning 17 total standards that ambulatory care nurses are accountable for.
Why the Field Is Expanding
Healthcare is moving out of the hospital. Procedures that once required overnight stays, from joint replacements to certain heart interventions, now happen in outpatient surgery centers. Bariatric surgical volumes in outpatient settings alone are expected to increase 13% in the coming years. Behavioral health is following the same trajectory, with outpatient volumes projected to grow 26% over the next decade.
This shift creates enormous demand for nurses who understand outpatient care delivery. The skills are different from acute care nursing: more emphasis on communication, education, and coordination; less emphasis on bedside monitoring and emergency intervention. For nurses entering the field or considering a transition, ambulatory care offers a career path that aligns with where healthcare is clearly headed.
