Unlike a disc herniation, where the inner nucleus pulposus breaks through a tear, a shallow bulge means the outer fibrous ring remains intact, though it may be stretched or weakened.

If the bulging disc puts pressure on nearby nerve roots, individuals might experience radiating pain, numbness, or tingling. For example, a bulge in the lower back (lumbar spine) can cause pain in the buttocks, legs, or feet (sciatica), while a neck bulge (cervical spine) might cause pain, numbness, or tingling in the neck, shoulders, arms, or hands. Symptoms can also worsen with certain movements, coughing, or sneezing.

Diagnosing a shallow disc bulge typically involves a thorough physical examination by a healthcare provider, who will assess pain, muscle reflexes, sensation, and muscle strength. Imaging tests play a central role in confirming the diagnosis and determining the extent of the disc changes. Magnetic Resonance Imaging (MRI) is particularly effective, providing detailed views of the discs and any potential compression of nerves. X-rays and Computed Tomography (CT) scans may also be utilized to evaluate the spinal structure.

Management of shallow disc bulges primarily focuses on conservative treatments, aiming to alleviate symptoms and improve function. Initial approaches often include rest from aggravating activities, along with pain management strategies such as nonsteroidal anti-inflammatory drugs (NSAIDs) or, for more severe pain, prescription medications. Physical therapy is a common and effective intervention, involving exercises and stretches designed to strengthen back muscles, improve flexibility, and restore range of motion. Other conservative measures can include heat or cold therapy, chiropractic care, and lifestyle modifications like maintaining a healthy weight and practicing good posture. Surgical intervention is rarely necessary for shallow disc bulges and is typically considered only if conservative treatments fail to provide relief after an extended period, or if there are signs of progressive neurological deficits.
