What Is a Practical Nurse? LPN Duties and Career Path

A practical nurse is a licensed healthcare professional who provides basic patient care under the supervision of a registered nurse or physician. Known formally as a Licensed Practical Nurse (LPN) in most states, or a Licensed Vocational Nurse (LVN) in Texas and California, this role sits between a nursing assistant and a registered nurse in terms of training, responsibility, and independence. Practical nurses are one of the fastest entry points into a nursing career, requiring about one year of education before entering the workforce.

What a Practical Nurse Does

Practical nurses handle a broad range of hands-on patient care. Their scope of practice, as defined by state nursing laws, includes caring for the ill, injured, or infirm; administering medications and treatments prescribed by a provider; teaching patients about preventive health measures; and acting to safeguard life and health. In day-to-day terms, that translates to checking vital signs, changing wound dressings, inserting catheters, helping patients with bathing and mobility, giving injections, and monitoring how patients respond to treatment.

Practical nurses also take on some coordination responsibilities. They can direct and assign nursing tasks to other licensed or unlicensed healthcare workers, manage nursing workflows, and carry out delegated medical functions. The key limitation is that all of this happens under supervision. A practical nurse doesn’t independently create a patient’s care plan or make diagnostic decisions, but they play an active role in carrying that plan out and flagging changes in a patient’s condition.

How LPNs Differ From RNs

The clearest distinction between a practical nurse and a registered nurse is the level of clinical judgment each role is authorized to exercise. An RN performs comprehensive, ongoing assessments: collecting and analyzing data, forming nursing diagnoses, and building a full care plan with prioritized goals and tailored interventions. A practical nurse participates in assessment by collecting data using structured guidelines and recognizing when immediate intervention is needed, but the deeper analysis and planning responsibility stays with the RN.

When it comes to implementing care, a practical nurse carries out the established plan rather than designing it. They can suggest goals and interventions to the supervising RN, but the RN holds final authority over the care strategy. RNs also have broader prescriptive abilities depending on the state, including starting IV medications and performing more complex procedures that fall outside an LPN’s training. In practice, the two roles work as a team: the RN sets the direction, and the LPN helps execute it while reporting back on how the patient is doing.

Education and Licensing

Becoming a practical nurse takes about 12 months. Programs are offered through community colleges and technical schools and combine classroom instruction with clinical rotations. Coursework covers anatomy, pharmacology, nutrition, and fundamental nursing skills, while clinical hours give students supervised experience with real patients. The relatively short timeline compared to RN programs (which take two to four years) makes practical nursing an appealing option for people who want to start working in healthcare quickly.

After completing a state-approved program, graduates must pass the NCLEX-PN, the national licensing exam for practical nurses. The exam uses computerized adaptive testing, meaning the difficulty of questions adjusts based on how you answer. Registration is a multi-step process that involves both your state’s nursing regulatory body and the testing vendor, Pearson VUE. Each state sets its own specific requirements, so it’s worth checking with the board of nursing in the state where you plan to practice before you begin.

Where Practical Nurses Work

The largest share of practical nurses, 37%, work in nursing and residential care facilities. This makes sense given that long-term care settings rely heavily on the kind of consistent, hands-on patient monitoring LPNs are trained to provide. Hospitals employ about 16% of LPNs, while home healthcare services and physicians’ offices each account for 12%. Government agencies outside of hospitals and schools employ another 6%.

The work environment shapes what your day looks like. In a nursing home, you might manage medications and wound care for a set group of residents over months or years, building ongoing relationships. In a hospital, the pace is faster and the patient mix changes constantly. In home health, you’re often working one-on-one with a patient in their own home, which requires more independence in managing your time and communicating updates to the supervising nurse or physician remotely. Physicians’ offices tend to offer more predictable hours, with duties that lean toward taking vitals, assisting with exams, and patient education.

Career Growth and LPN-to-RN Programs

Many practical nurses treat the role as a stepping stone toward becoming a registered nurse. LPN-to-RN bridge programs are specifically designed for this transition, and they’re shorter than starting an RN program from scratch. At Johnson County Community College, for example, the bridge program spans three semesters (about one year), running from June through May. Students who complete the initial bridge courses automatically receive credit for foundational nursing courses they’ve already mastered through their LPN training and experience.

These programs typically require 21 to 24 prerequisite credits before you can enroll in the bridge-specific coursework, bringing the total to roughly 56 to 59 credit hours. After finishing, graduates are eligible to sit for the NCLEX-RN. The practical benefit of this path is significant: you can work as an LPN and earn a paycheck while completing prerequisites, then enter a condensed program that builds on skills you already use every day. For nurses who want to expand their scope of practice, increase their earning potential, or move into leadership roles, the bridge pathway is one of the most efficient routes available.

LPN vs. LVN: The Naming Difference

If you’ve seen the terms LPN and LVN used interchangeably, that’s because they refer to the same role. Texas and California use the title Licensed Vocational Nurse, while every other state uses Licensed Practical Nurse. The training, licensing exam, and scope of practice are essentially the same. If you move between states, you’ll need to apply for licensure in your new state regardless of the title, but the credential itself transfers.
