A PN in nursing stands for practical nurse, a licensed healthcare professional who provides basic medical care under the supervision of a registered nurse or physician. In most U.S. states, a practical nurse holds the title Licensed Practical Nurse (LPN). In Texas and California, the same role goes by Licensed Vocational Nurse (LVN). The job duties are identical regardless of which title is used.

What a Practical Nurse Does

Practical nurses handle hands-on patient care that keeps clinical settings running day to day. Their scope of practice includes administering most types of medications, immunizations, and some blood products. They perform clinical procedures like urinary catheterizations, tracheal suctioning, sterile dressing changes, and starting peripheral IVs. They also observe, measure, record, and report data related to a patient’s health status.

All of this work happens under the direction of a registered nurse, nurse practitioner, physician, or another qualified practitioner. PNs are task-driven: they carry out treatment plans rather than creating them. An RN might assess a patient and design a care plan, and the PN executes that plan at the bedside. This distinction is the core legal and practical difference between the two roles.

Where Practical Nurses Work

Skilled nursing facilities employ more PNs than any other setting, with over 171,000 working in those facilities as of May 2023. General hospitals come next at roughly 84,900, followed closely by home health care services (78,100) and physicians’ offices (74,850). Continuing care retirement communities and assisted living facilities round out the top five at about 41,800.

This distribution reflects the nature of the role. Long-term care and elder care settings need steady, reliable bedside nursing around the clock, and PNs fill that need. In hospitals, PNs typically work on medical-surgical floors rather than in intensive care or emergency departments, where the complexity of patient assessment requires RN-level training.

Education and Licensing

Becoming a practical nurse is one of the fastest paths into a nursing career. PN training programs, offered through community colleges and technical schools, typically run about 12 months. The curriculum covers fundamental nursing skills, pharmacology, anatomy, and patient care, with a mix of classroom instruction and supervised clinical rotations.

After completing the program, graduates must pass the NCLEX-PN, a national licensing exam. Every state requires this exam before a practical nurse can begin working. The license must be maintained through continuing education, with specific renewal requirements varying by state.

How PNs Differ From RNs

The biggest difference comes down to scope and autonomy. Registered nurses conduct patient assessments, create care plans, and record medical histories. They can work more independently and take on leadership roles within a care team. Practical nurses work within the framework RNs establish, focusing on direct patient tasks rather than clinical decision-making.

Education is the other major gap. RN programs require either a two-year associate degree or a four-year bachelor’s degree, compared to the one-year certificate for PNs. That additional training gives RNs the clinical judgment skills needed for more complex patient situations, and it’s reflected in higher earning potential and a broader range of work settings.

Pay and Employment

Compensation for practical nurses varies significantly by setting. In May 2023, PNs working in skilled nursing facilities earned a mean annual wage of about $63,700, while those in physicians’ offices averaged around $53,600. Home health care and assisted living positions fell in between, with mean wages near $61,000. Hospital-based PNs averaged roughly $55,400.

The pay gap between settings partly reflects shift differentials and overtime availability. Skilled nursing facilities often operate 24/7 with high demand for evening and weekend coverage, which pushes total compensation higher.

Moving From PN to RN

Many practical nurses treat the role as a stepping stone. LPN-to-RN bridge programs are designed specifically for working PNs who want to advance without starting nursing school from scratch. These programs typically take about 11 months and award an associate degree in nursing upon completion.

Admission requirements generally include completing prerequisite courses in anatomy, physiology, microbiology, psychology, nutrition, and English composition, all with a grade of C or better. Some programs also require a minimum score on a nursing entrance exam. Once you finish a bridge program, you sit for the NCLEX-RN to earn your registered nurse license. The transition lets you build on bedside experience you already have while gaining the assessment and care planning skills that expand your scope of practice.
