Palpation is formally defined as the process of using one’s hands to physically examine a part of the body, perceiving specific characteristics that are otherwise undetectable. The primary goal is to assess the physical state of underlying organs, tissues, and structures, whether superficial or deep. Practitioners use their hands and fingertips to gather sensory input about the area being examined, which helps guide further investigation and treatment.
The sensory information gathered involves several characteristics of the tissues. These include the texture of the skin or underlying mass, the size and shape of an abnormality, and its consistency. The examination also assesses temperature, moisture, and the motility of structures, such as a mass that can be easily moved or one that is fixed in place. Detecting a pulse or heartbeat is also considered a form of palpation, as it involves feeling for rhythm and force.
Healthcare providers develop a refined skill set to recognize subtle tactile signs. This mastery allows them to evaluate not only static features but also dynamic responses, such as muscle guarding or involuntary tensing when an area is touched. Deep palpation can be performed with one hand, or using a bimanual technique where one hand applies the pressure while the other focuses on feeling the structures. This two-handed approach is useful for outlining the size and shape of a suspected mass or organ.

A primary application is the detection of abnormal growths or masses, such as tumors, ovarian cysts, or lipomas, characterized by their location, size, and shape. The technique is also used to check for hernias, where an internal body part protrudes through a weakness in the muscle wall.
Assessing the patient’s pain response is another significant application, specifically through testing for tenderness, which signals underlying inflammation or irritation. For instance, rebound tenderness, where pain increases upon the quick release of deep pressure, can suggest an acute abdominal process like appendicitis. Palpation is also used to determine the size and position of organs, such as feeling for an enlarged liver or spleen, which may indicate systemic diseases. The sense of touch allows the provider to assess circulation by checking the strength and rhythm of a pulse and to detect localized skin temperature changes that suggest inflammation or infection.
