The Role of Lipase in the Body

Lipase is primarily a fat-splitting enzyme that acts on triglycerides, the main form of fat found in food. This enzyme hydrolyzes these large fat molecules into smaller components, specifically free fatty acids and glycerol. These smaller molecules can then be absorbed by the intestines and used by the body for energy or storage.

The majority of the lipase measured in the blood originates from the pancreas, an organ situated behind the stomach. Pancreatic cells produce this enzyme and release it into the small intestine through a duct to facilitate digestion. While the pancreas is the main source, smaller amounts of lipase are also produced by the salivary glands and the stomach.

Understanding the Lipase Blood Test

A lipase blood test, often called a serum lipase test, measures the amount of this enzyme circulating in the blood. A healthcare provider typically orders this test when a person exhibits symptoms suggesting a problem with the pancreas, such as severe abdominal pain, nausea, vomiting, or a rapid heart rate. Elevated levels of lipase are particularly indicative of pancreatic issues.

The procedure involves a standard blood draw from a vein in the arm. Since certain medications and foods can influence the enzyme’s level, fasting for eight to twelve hours before the test may be required. It is important to tell your doctor about all prescription and over-the-counter drugs, as some, like codeine or certain diuretics, can affect the result.

Defining the Normal Lipase Range

Lipase levels are measured in units per liter (U/L) of blood. While the exact “normal” range can vary slightly, a commonly accepted range for an adult is approximately 0 to 160 U/L. Some laboratories may report a range of 0 to 140 U/L, or a slightly different range depending on the testing method used.

The reference interval is not universal and depends heavily on the specific laboratory performing the analysis. The normal range for adults over 60 might be slightly different than for younger individuals. Consequently, a patient’s result must always be compared against the specific reference range printed on their lab report to determine if it is normal.

Interpreting Abnormal Lipase Levels

Results outside the normal range suggest the pancreas may be inflamed or damaged, causing the enzyme to leak into the bloodstream. Elevated lipase levels are most commonly linked to acute pancreatitis, a sudden inflammation of the pancreas. In these cases, the lipase concentration often rises significantly, typically reaching three to ten times the upper limit of the normal range. The enzyme level rises quickly after injury and may remain elevated for a week or more, making it a reliable diagnostic marker.

High lipase levels are not exclusive to acute pancreatitis and can also be observed in other conditions. These include chronic pancreatitis, kidney failure, intestinal obstruction, diabetic ketoacidosis, or a perforated peptic ulcer. Therefore, a doctor will always consider the lipase result alongside the patient’s symptoms and other blood tests before making a diagnosis.

Conversely, a lower-than-normal lipase level is less common and is generally not as clinically revealing as an elevated level. A low reading can sometimes indicate long-term damage to the pancreas that has reduced the organ’s ability to produce the enzyme. Conditions such as chronic pancreatitis or cystic fibrosis, which cause progressive damage and scarring, are associated with diminished lipase production.
