This method is used because the precise date of conception is often unknown, while the start of a menstrual period is usually a more identifiable event.

Gestational age starts counting approximately two weeks before conception, serving as the standardized clinical measure for pregnancy duration. In contrast, “fetal age” refers to the actual age of the fetus from conception, which is generally about two weeks less than gestational age. Healthcare providers rely on gestational age for consistency in medical assessments and scheduling.

Methods of Calculation

The initial and most common way to estimate gestational age is using the first day of the last menstrual period (LMP). This method assumes a regular 28-day menstrual cycle with ovulation around day 14. However, this approach has limitations if the individual has irregular cycles, cannot recall their LMP accurately, or conceived while using hormonal contraceptives.

When the LMP is uncertain, an early ultrasound is often performed for a more accurate estimation. Ultrasound measurements, particularly the crown-rump length (CRL) in the first trimester (between 6 and 13 weeks), offer a precise way to determine gestational age. The CRL measures the length of the embryo or fetus from head to buttocks and is considered the most accurate method for dating early pregnancy due to minimal biological variability at this stage.

If a significant discrepancy exists between the LMP and early ultrasound measurement, the ultrasound-derived gestational age is typically used as the more reliable estimate. Knowing gestational age is fundamental for estimating the baby’s due date, typically around 40 weeks from the LMP. While only about 4% of babies are born precisely on their estimated due date, this calculation provides a valuable timeframe for parents and medical teams. An accurate due date helps manage expectations and prepare for labor and delivery.
