What Is a CRN Nurse? Role, Certification, and Career Path

A CRN, or Certified Radiology Nurse, is a registered nurse who has earned a specialty credential in radiology nursing by passing a certification exam through the Radiologic Nursing Certification Board (RNCB). These nurses care for patients before, during, and after imaging and interventional radiology procedures, working in settings that range from MRI suites and CT labs to interventional radiology operating rooms.

What a CRN Does Day to Day

Radiology nurses bridge the gap between the patient and the radiologist. Their work centers on keeping patients safe and comfortable during procedures that most people find unfamiliar or intimidating. In a typical shift, a CRN might prepare a patient for a contrast-enhanced CT scan, monitor vitals during an interventional procedure, administer sedation, or manage allergic reactions to imaging dyes.

In interventional radiology specifically, the role looks more like surgical nursing. CRNs assist radiologists during angiographic and interventional procedures by preparing sterile supply trays, passing guide wires and catheters, and maintaining sterile technique throughout the case. The procedures themselves vary widely: biopsies, vascular interventions, catheter placements, and both neurological and vascular angiography. Between procedures, these nurses handle patient assessments, IV access, medication administration, and post-procedure recovery monitoring.

Where CRNs Work

Most CRNs work in hospitals, but the specific departments they’re assigned to cover a broad range of radiology subspecialties. At large medical centers like Mayo Clinic, radiology nurses work across areas including abdominal and ultrasound imaging, breast imaging, cardiovascular and thoracic radiology, interventional radiology, neuroradiology, nuclear medicine, and pediatric imaging. Each of these areas comes with its own patient populations and procedural demands.

Outside of major hospitals, CRNs also work in outpatient imaging centers, ambulatory surgery centers that perform image-guided procedures, and community health systems. Some take on roles in education, clinical management, or supervision of other staff rather than direct patient care.

How to Become a Certified Radiology Nurse

The CRN credential isn’t something a new nurse can pursue right out of school. To sit for the certification exam, you need to meet three requirements simultaneously:

  • Active RN license: You must hold a current registered nurse license (or international equivalent). Hours worked as an LPN, LVN, or graduate nurse don’t count toward eligibility.
  • 1,500 hours of radiology nursing experience: These hours must have been completed within the past three years. They can come from direct patient care or from clinical management, supervision, education, or direction of others in a radiology setting.
  • 30 continuing education hours: These must be completed within 24 months of your exam date and must relate to nursing care of radiology patients. At least 15 of those 30 hours need to be specifically focused on radiology nursing topics.

For context, 1,500 hours is roughly equivalent to working full-time in a radiology department for about nine months. Nurses who work part-time in radiology can still qualify, but they’ll need more calendar time to accumulate the required hours.

The Certification Exam

The CRN exam is administered by the RNCB and covers nursing care in imaging, interventional, and therapeutic radiology environments. The passing threshold requires answering at least 73% of questions correctly. The exam tests knowledge across the full scope of radiology nursing practice, so candidates benefit from experience in more than one subspecialty area.

The board members who set the passing score work actively in radiology nursing across a variety of settings, which keeps the exam aligned with current clinical practice rather than outdated textbook material.

Why the Certification Matters

Holding a CRN credential signals specialized expertise to employers and patients. Radiology nursing involves skills that general medical-surgical nurses typically don’t develop, including contrast media management, radiation safety awareness, sedation monitoring for image-guided procedures, and familiarity with the equipment and workflow of radiology departments.

From a career standpoint, specialty certification often translates to higher compensation. Many hospitals factor certifications into their clinical ladder programs, which tie pay increases and advancement opportunities to demonstrated expertise. The CRN also satisfies continuing competency requirements in some states. In Texas, for example, nurses can use the achievement or renewal of a board-approved national certification to meet their license renewal requirements in place of completing separate continuing education hours.

CRN vs. Other Nursing Certifications

The nursing field has dozens of specialty certifications, and it’s easy to confuse acronyms. The CRN is specifically for radiology nursing and is issued by the RNCB. It’s distinct from certifications like CCRN (critical care), CEN (emergency nursing), or CNOR (operating room nursing), even though radiology nurses may overlap with some of those environments during interventional procedures.

If you’re a nurse considering radiology as a specialty, the CRN is the only credential that specifically validates your knowledge in this field. If you’re a patient, seeing “CRN” after your nurse’s name means they’ve met a standardized benchmark of radiology-specific training and experience beyond their basic nursing license.
