What Is a CNA License? Duties, Exam, and Pay

A CNA license is a state-issued credential that allows you to work as a Certified Nursing Assistant, providing hands-on patient care under the supervision of registered nurses or licensed practical nurses. It’s one of the fastest entry points into healthcare, with most people completing training and earning certification in a matter of weeks. Every state requires CNAs to pass a competency exam and be listed on an official registry before they can work in licensed healthcare facilities.

What a CNA Actually Does

CNAs handle the daily, physical side of patient care that nurses don’t have time for. Their core responsibilities center on helping patients with activities of daily living: bathing, dressing, using the toilet, eating meals, and moving between beds and wheelchairs. They also measure vital signs like temperature and blood pressure, listen to patients’ health concerns, and relay that information to the nursing team.

The scope of what a CNA can do varies by state. Some states allow CNAs with additional training to dispense medication, while others restrict that entirely. Each state publishes an official skills list, and CNAs are prohibited from performing any task not on it, regardless of whether they feel capable. If a task isn’t on the approved list, it falls outside the CNA’s legal scope of practice. This is a hard boundary, not a suggestion.

Most CNAs work in nursing homes, hospitals, assisted living facilities, and home health settings. The work is physically demanding and emotionally rewarding in equal measure. You’re often the person who spends the most face-to-face time with patients, which makes the role essential to how people experience their care.

How to Get Certified

The path to a CNA license has three steps: complete an approved training program, pass a state competency exam, and get placed on your state’s Nurse Aide Registry.

Federal law, established through the Omnibus Budget Reconciliation Act of 1987, requires a minimum of 75 hours of training. Many states exceed this minimum significantly. Programs typically combine classroom instruction with supervised clinical hours where you practice skills on real patients in a healthcare facility. Community colleges, vocational schools, and some nursing homes offer these programs, with timelines ranging from four to twelve weeks depending on the schedule.

Before you start a program, expect to meet a few prerequisites. Most require you to be at least 18 (though some states allow 16 or 17-year-olds), have a high school diploma or GED, and pass a background check. In California, for example, students must submit fingerprints for a Live Scan background check before any direct patient contact. A physical health exam is also standard, including a tuberculosis screening and documentation that you don’t have a health condition that would pose a risk to patients or coworkers. This exam must be completed before you begin clinical rotations.

The Certification Exam

After finishing your training program, you’ll take a two-part state competency exam. The first part is a written (or oral) knowledge test covering infection control, patient safety, communication, and basic nursing skills. The second part is a clinical skills evaluation where you demonstrate specific tasks in front of an evaluator, such as taking blood pressure, properly repositioning a patient, or performing hand hygiene.

Most states give you three attempts to pass. If you fail all three, you’ll typically need to retake the training program before testing again. The pass rate is generally high for students who complete their training, and many programs include practice exams to prepare you.

Cost and Time Investment

CNA training programs range widely in cost depending on where you live and who runs the program. At Florida State College at Jacksonville, for instance, the full program costs $1,228, split between a healthcare foundations course and the nursing assistant course itself. Across the country, prices typically fall between $600 and $2,000 for community college or vocational programs. Private training schools can charge more.

Some employers, particularly nursing homes facing staffing shortages, offer free CNA training in exchange for a commitment to work at their facility for a set period after certification. This can be a smart option if you’re watching costs, though read the terms carefully since leaving early may mean repaying tuition. Your state’s workforce development office or local Red Cross chapter may also offer reduced-cost programs.

Beyond tuition, budget for the state exam fee (usually $100 to $150), your background check, scrubs, and the physical health exam.

The State Nurse Aide Registry

Once you pass the exam, your name goes on your state’s Nurse Aide Registry. This registry is the backbone of the entire system. Any facility that receives Medicare or Medicaid funding is legally required to verify that a CNA is listed in good standing before hiring them. A CNA who isn’t enrolled or in good standing on the registry cannot legally work in these facilities.

The registry also tracks disciplinary actions. If a CNA is found guilty of patient abuse, neglect, or theft of a resident’s property, that finding is permanently noted on their file, effectively barring them from nursing home employment. Facilities can check any CNA’s status instantly through online registry portals, so there’s no way to sidestep a negative record by applying elsewhere.

Keeping Your Certification Active

CNA certifications don’t last forever. Most states require renewal every one to two years, and the main requirement is proof that you’ve worked as a CNA during that period. In Colorado, for example, you need at least eight hours of paid CNA work per renewal cycle to maintain your certification. If you let your certification lapse by not working long enough, you may need to retest or retrain.

Continuing education requirements vary by state. Some states mandate a specific number of in-service training hours each year, while others, like Colorado, don’t currently require continuing education for renewal at all. Your state’s board of nursing or health department website will have the exact requirements.

Pay and Job Outlook

The median annual wage for nursing assistants was $39,530 as of May 2024, according to the Bureau of Labor Statistics. Pay varies considerably by setting and location. CNAs in hospitals and metropolitan areas tend to earn more than those in rural nursing homes.

Employment growth is projected at 2 percent from 2024 to 2034, which is slower than average. That said, the sheer size of the workforce and consistent turnover mean tens of thousands of CNA positions open each year. Demand is especially strong in long-term care facilities as the population ages.

Using a CNA License as a Stepping Stone

Many CNAs use the role as a launchpad for further nursing careers. Working as a CNA gives you direct patient care experience that strengthens applications to LPN or RN programs. Some nursing schools give preference to applicants with CNA experience, and the clinical exposure helps you understand what nursing actually involves before committing to a longer degree.

With additional training, CNAs can also specialize. Some pursue certification as medication aides, home health aides, or patient care technicians, each of which expands the scope of what you’re allowed to do and typically comes with higher pay. The CNA credential itself, though, remains portable and recognized in every state, making it one of the most accessible healthcare licenses available.
