What Is a CNA Certification and How Do You Get One?

A CNA certification is a state-issued credential that qualifies you to work as a Certified Nursing Assistant, providing hands-on patient care under the supervision of nurses and doctors. It’s one of the fastest entry points into healthcare, with training programs that can be completed in as little as one semester. To earn the certification, you must complete a state-approved training program and pass a competency exam that includes both a written test and a practical skills evaluation.

What a CNA Actually Does

Certified Nursing Assistants handle the daily, physical side of patient care. According to the Bureau of Labor Statistics, their core duties include bathing and cleaning patients, helping with toileting and dressing, turning and repositioning patients, transferring them between beds and wheelchairs, serving meals, and assisting with eating. CNAs also measure vital signs like blood pressure and temperature, and they play a key communication role: listening to patients’ health concerns and reporting that information back to nurses.

In some states, CNAs with additional training are also authorized to dispense medication. The exact scope of what you can do varies by state, but across the board, CNAs are the healthcare workers who spend the most direct time with patients. You’re the person who notices when something changes, whether that’s a new complaint, a shift in mood, or an abnormal reading.

Training Requirements

Federal law sets a floor of 75 clock hours for any state-approved CNA training program. Within those hours, at least 16 must be supervised practical training, where you demonstrate skills on a real person under the direct watch of a registered nurse or licensed practical nurse. Before you’re allowed any direct contact with a resident, you also need at least 16 hours covering communication skills, infection control, safety and emergency procedures (including the Heimlich maneuver), promoting patient independence, and respecting residents’ rights.

Those are minimums. Many states require significantly more. Some set the bar at 120 or even 180 hours of training. In practice, most programs at community colleges and vocational schools run for one semester, combining classroom instruction with clinical rotations at a healthcare facility. The clinical portion is where you practice taking vitals, assisting with mobility, and performing basic care tasks in a real-world setting before you sit for the certification exam.

The Certification Exam

After completing your training program, you’ll take a two-part competency evaluation. The first part is a written (or oral) exam testing your knowledge of patient care, safety procedures, and basic medical concepts. The second part is a skills demonstration, where a nurse evaluator watches you perform specific tasks like handwashing, taking blood pressure, or repositioning a patient in bed. You typically need to pass both parts to earn your certification.

Each state contracts with a testing company to administer the exam. If you fail one section, most states allow you to retake it without repeating the entire test, though policies on the number of attempts vary.

Cost and Time Investment

CNA training is one of the more affordable healthcare credentials. Program costs vary widely depending on location and institution, but community college programs often fall in the range of $1,000 to $2,000 for tuition and fees. At Edmonds College in Washington state, for example, the total program cost comes to roughly $1,190 in tuition plus around $380 in book fees and about $45 for scrubs. Additional costs include the certification exam fee and a criminal background check, which is required in every state.

Some employers, particularly nursing homes facing staffing shortages, offer free CNA training in exchange for a commitment to work at their facility for a set period after certification. Community organizations and workforce development programs also sometimes cover training costs. If you’re price-sensitive, it’s worth checking whether a local long-term care facility has a sponsored program before paying out of pocket.

Where CNAs Work

Nursing homes and long-term care facilities are the largest employers of CNAs, but they’re far from the only option. CNAs also work in hospitals, assisted living communities, rehabilitation centers, home health agencies, and outpatient clinics. The setting shapes your daily experience significantly. In a nursing home, you’ll typically care for the same group of residents over weeks or months, building relationships. In a hospital, patient turnover is faster, the pace is more intense, and you may rotate between departments.

Home health is a growing segment of the field. Working in a patient’s home means more autonomy and one-on-one time, but it also means less immediate access to a supervising nurse if questions arise.

Moving Your Certification to Another State

CNA certification is state-specific, so if you relocate, you’ll need to transfer your credential through a process called endorsement or reciprocity. The general requirements are consistent: your current certification must be active and in good standing, and you’ll need to submit verification from your original state’s licensing agency. You’ll also need to pass a new background check, which typically involves electronic fingerprinting through an approved provider.

Florida’s process is a good illustration of what to expect. The state board attempts to verify your license online first. If online records aren’t detailed enough, you’ll need to request an official verification mailed directly from your original state’s licensing agency. If your home state doesn’t track employment records to validate “active” status, you may also need to submit employment verification from a recent employer. The process can take several weeks, so it’s smart to start well before your planned move.

If your certification has lapsed or you don’t hold an active license anywhere, most states require you to retake the competency exam rather than simply reactivating your credential.

CNA as a Career Stepping Stone

Many people pursue CNA certification not as a final destination but as a way to enter healthcare quickly and build experience while deciding on a longer-term path. Working as a CNA gives you direct exposure to patient care, hospital workflows, and the realities of nursing, all of which are valuable if you’re considering becoming a licensed practical nurse, registered nurse, or pursuing another clinical role.

Nursing school admissions committees generally view CNA experience favorably, and many CNA-to-RN bridge programs exist at community colleges. Some large hospital systems offer tuition assistance for CNAs pursuing further education. The certification itself requires periodic renewal, usually every two years, with proof of recent employment in a nursing role and sometimes a minimum number of continuing education hours depending on your state.
