Taking levothyroxine with food reduces how much of the drug your body actually absorbs, which can leave you undertreated for hypothyroidism. A higher TSH means your body is signaling that it needs more thyroid hormone than it’s getting.

The effect is statistically significant even without any food present. Coffee doesn’t bind the drug as aggressively as bran or antacids do, but it still meaningfully reduces how much medication gets through.

What Inconsistent Absorption Feels Like

That full hour gives the drug time to pass through your stomach and begin absorbing in the small intestine before food arrives and disrupts the process. Some people manage well with a 30-minute gap, but the evidence is strongest for a full 60 minutes.
Supplements need even more separation. Calcium supplements should be taken at least four hours before or after your levothyroxine. Iron supplements follow the same rule. These minerals are some of the most potent blockers of levothyroxine absorption, and even a modest overlap in timing can make a measurable difference in your thyroid levels.
Bedtime Dosing as an Alternative
If waiting an hour before breakfast feels impractical, taking levothyroxine at bedtime is a viable option. The key requirement is waiting at least two to three hours after your last meal. One randomized controlled study found that bedtime dosing actually decreased TSH levels by 1.25 points compared to morning dosing, suggesting absorption may even be better at night when the gut has been empty for hours.
Results have been mixed across studies, though.
