Preparation H reduces hemorrhoid symptoms by shrinking swollen tissue, relieving itch, and soothing irritated skin around the anus. It’s an over-the-counter treatment, not a cure. It won’t make hemorrhoids disappear permanently, but it can provide meaningful short-term relief from the pain, burning, and swelling that make hemorrhoids so uncomfortable.

How It Works in Your Body

The core ingredient in most Preparation H products is phenylephrine, a vasoconstrictor. It works by activating receptors on the walls of blood vessels that cause them to tighten. Since hemorrhoids are essentially swollen, engorged blood vessels in and around the rectum, constricting those vessels reduces the swelling and takes pressure off the surrounding tissue. That’s what eases the pain and the feeling of fullness or pressure.

Some formulations also contain witch hazel, a plant-based astringent that tightens tissue and decreases inflammation. Witch hazel targets a slightly different set of symptoms: the itching, burning, and surface irritation that come from inflamed skin, especially after bowel movements. A few Preparation H products contain hydrocortisone, a mild steroid that specifically tackles intense itching by calming the immune response in the skin.

What Each Product Type Is For

Preparation H comes in several forms, and they’re not interchangeable. Each one is designed for a different situation.

  • Creams and ointments are the most common. They’re applied externally up to four times a day, typically in the morning, at night, and after bowel movements. These treat external hemorrhoid symptoms like swelling, pain, and itch on the skin’s surface.
  • Suppositories are designed for internal hemorrhoids, the ones that form inside the rectum. They use phenylephrine to shrink swollen tissue from the inside, providing relief from pressure and discomfort that topical creams can’t reach.
  • Medicated wipes contain witch hazel and work as a gentle, soothing cleanser. They’re most useful for reducing irritation during and after bowel movements, rather than for treating swelling directly.
  • Hydrocortisone cream is a separate formulation specifically for external anal itching. It’s not meant to be inserted into the rectum. The steroid component makes it effective for persistent itch but also means it should be used more cautiously than the standard versions.

What to Expect When Using It

Phenylephrine works relatively quickly once applied, since vasoconstriction is an almost immediate physical response. You’ll likely notice reduced swelling and pressure within minutes. The relief is temporary, though, which is why reapplication up to four times daily is recommended.

The key benchmark to keep in mind is seven days. If your symptoms haven’t improved within a week of consistent use, or if they get worse at any point, that’s a signal to see a doctor. Safety Considerations

If you have high blood pressure or heart disease, choose a phenylephrine-free formulation, such as one that relies on witch hazel or a protectant instead.

The hydrocortisone version carries its own set of concerns. Hydrocortisone is a steroid, and prolonged use on any skin surface can lead to thinning, fragility, and easy bruising. This risk is especially pronounced on delicate skin. Using it for longer than the label recommends, or applying it to areas it’s not intended for, increases the chance of these side effects.

Using It on Your Face

There’s a persistent idea that Preparation H can reduce under-eye puffiness and wrinkles. The logic isn’t entirely wrong: phenylephrine does constrict blood vessels, which could temporarily make puffy skin look tighter. Some dermatologists acknowledge this effect. But no clinical studies have demonstrated that hemorrhoid cream actually reduces wrinkles or improves facial skin.

More importantly, the risks outweigh any cosmetic benefit. Prolonged use of phenylephrine around the eyes can leave skin thinner, more fragile, and prone to redness and swelling, the opposite of what you’re going for. If you’re dealing with recurring hemorrhoids, the long-term fix involves dietary fiber, adequate water intake, and avoiding straining. Preparation H is the relief you use in the meantime while those changes take effect, or during a flare-up that makes daily life uncomfortable.
