Tropicamide is a frequently used anticholinergic agent, while atropine is a stronger, longer-acting option, sometimes used in children or for specific therapeutic purposes. The choice of dilating drop depends on factors such as the patient’s age, eye color, medical history, and the specific reason for the dilation.
How Dilating Drops Affect Your Eyes
Dilating drops interact with the muscles controlling the pupil to achieve a wider opening. The iris contains two key muscles: the sphincter muscle, which constricts the pupil, and the dilator muscle, which widens it. Adrenergic agonists like phenylephrine stimulate the dilator muscle, causing it to contract and pull the pupil open.
Anticholinergic drops, such as tropicamide and atropine, block acetylcholine, a neurotransmitter that normally causes the sphincter muscle to contract and the ciliary muscle to focus. By blocking these receptors, anticholinergics cause the sphincter muscle to relax, leading to pupil dilation. This action also induces cycloplegia, the temporary paralysis of the ciliary muscle, which prevents the eye from accommodating or changing focus. This allows the doctor to obtain an accurate measurement of refractive error and a clearer, unobstructed view of the retina.
Why Eye Dilation is Necessary

Experiencing Eye Dilation
During an eye exam, dilating drops are applied to each eye. Pupils typically take 20 to 30 minutes to fully widen, though some drops begin working within 10 to 15 minutes. You may experience a temporary stinging sensation when the drops are administered.
After dilation, common temporary side effects include increased sensitivity to light and blurry vision, especially for close-up tasks. The effects of dilation usually last for about four to six hours, but this can vary depending on the specific drops used, their concentration, and individual factors such as eye color; lighter-colored eyes may experience longer dilation. For children, the effects can last up to a full day. It is advisable to bring sunglasses to manage light sensitivity. Because vision may be blurry, especially for driving, arranging for someone to drive you home is often recommended. Avoiding close work and bright lights until the effects wear off can enhance comfort during this temporary period.
