What Are Quality Indicators in Healthcare?

Quality indicators are standardized, evidence-based measures used to track how well an organization performs. While the term applies across many industries, it’s most developed in healthcare, where quality indicators measure clinical performance, patient safety, and health outcomes using data that hospitals and clinics already collect. They give patients, administrators, and policymakers a way to compare providers, spot problems, and drive improvement with numbers rather than guesswork.

How Quality Indicators Are Categorized

The most widely used framework for organizing quality indicators comes from a physician and researcher named Avedis Donabedian, who proposed three categories: structure, process, and outcome. Nearly every quality measurement system in healthcare builds on this model.

Structure indicators measure the resources and systems an organization has in place. Think of these as the foundation: whether a hospital uses electronic medical records, the ratio of providers to patients, or the proportion of board-certified physicians on staff. Structure doesn’t tell you whether patients are getting better, but it signals whether the conditions for good care exist.

Process indicators measure what providers actually do. These reflect accepted clinical recommendations, like the percentage of patients receiving preventive screenings (mammograms, immunizations) or the percentage of people with diabetes who had their blood sugar tested and controlled. Process measures are the most commonly reported type of quality indicator in public reporting systems because they’re directly within a provider’s control.

Outcome indicators measure what happens to patients as a result of care. Surgical mortality rates, hospital-acquired infection rates, and rates of surgical complications all fall here. Outcomes might seem like the gold standard, but they’re shaped by many factors beyond a provider’s control, including how sick patients were before treatment began. That’s why outcomes are rarely used in isolation.

Major Quality Indicator Systems in Healthcare

In the United States, the Agency for Healthcare Research and Quality (AHRQ) maintains one of the most prominent sets of quality indicators. These are designed to work with routine hospital administrative data, meaning they don’t require expensive new data collection. AHRQ organizes its indicators into four modules:

  • Prevention Quality Indicators (PQI): Flag hospitalizations that could have been avoided with better outpatient care, like admissions for uncontrolled diabetes or asthma.
  • Inpatient Quality Indicators (IQI): Measure hospital-level performance on procedures like hip replacements or heart bypass surgery.
  • Patient Safety Indicators (PSI): Track potentially preventable complications, such as infections after surgery or accidental punctures during procedures.
  • Pediatric Quality Indicators (PDI): Apply similar logic to children’s hospital care.

Globally, the World Health Organization publishes core health indicators that member states use to monitor health system performance at a national level. In clinical laboratories, the ISO 15189 standard requires labs to establish quality indicators for each phase of testing: pre-analytical (before the test is run), analytical (during testing), and post-analytical (after results are generated). The standard doesn’t prescribe exactly which indicators to use, but it requires labs to define goals, set limits, and review indicators periodically.

Nursing care has its own set of indicators as well. The National Database of Nursing Quality Indicators tracks measures like patient fall rates, pressure injury rates, catheter-associated urinary tract infections, and central line-associated bloodstream infections. Research using this database has shown, for instance, that nurse staffing levels and skill mix are directly associated with patient falls and pressure injuries, and that hourly rounding reduces fall rates.

What Makes an Indicator Valid

Not every number you can measure qualifies as a useful quality indicator. Researchers have identified several criteria an indicator needs to meet before it’s worth tracking. An ideal indicator is valid, reliable, sensitive, specific, and feasible.

Validity means the indicator actually reflects the quality of care it claims to measure. For a clinical indicator, this typically requires three things: scientific evidence linking that indicator to better patient outcomes, agreement that a provider with higher adherence would be considered higher quality, and confidence that the provider (not outside factors) controls most of what determines performance on that measure.

Feasibility is equally important and often overlooked. An indicator is feasible if the information needed to calculate it is already available in medical records or can be gathered through patient surveys at reasonable cost. If collecting the data imposes a heavy burden on clinicians or requires building expensive new systems, the indicator may not be practical regardless of how valid it is.

How Indicators Are Calculated and Compared

At their simplest, quality indicators are ratios. You take the number of times something happened (the numerator) and divide it by the number of times it could have happened (the denominator). The percentage of surgical patients who developed an infection, for example, divides infection count by total surgeries.

The real complexity comes in making fair comparisons between providers. A hospital that treats sicker patients will naturally have worse raw outcomes, so benchmarking methods often include risk adjustment to account for patient complexity. Two common approaches exist in surgical quality measurement. The simpler “75th percentile” method defines the benchmark as the 75th percentile of median results across participating centers. This represents performance far above average but still realistically achievable, and it doesn’t require formal risk adjustment because it focuses on selecting comparable patient groups. The more sophisticated “Achievable Benchmark of Care” method identifies the top 10% of providers by patient volume, adjusts their performance for risk factors, and uses that subset’s pooled results as the target.

Without risk adjustment, comparing raw outcome numbers between a community hospital and a major trauma center would be misleading. The trauma center’s patients are sicker on arrival, so its complication rates will naturally be higher even if its care is excellent.

The Cost of Measuring Quality

Quality indicators only work if the data behind them is accurate, and collecting that data takes real time. A study of healthcare professionals found they spend an average of 52.3 minutes per working day on quality-related registrations. That’s nearly an hour of every shift devoted to documentation rather than direct patient care.

This burden creates its own problems. When clinicians feel overwhelmed by documentation demands, several things go wrong: they may resort to “autopilot box-ticking,” entering data reflexively without ensuring accuracy. Double registrations pile up when multiple reporting systems demand similar but slightly different information. IT inefficiencies compound the frustration. And some registrations simply don’t feel feasible to complete during the normal flow of clinical work.

Beyond the time cost, poorly designed indicator systems can produce unintended consequences. Measurement fixation leads organizations to focus narrowly on what’s being measured while ignoring equally important areas that aren’t tracked. Misplaced incentives can push providers toward gaming behaviors, optimizing their numbers without actually improving care. A hospital might, for example, avoid operating on the sickest patients to keep its mortality statistics low.

These risks don’t mean quality indicators are a bad idea. They mean the choice of which indicators to track, how data is collected, and how results are used all matter enormously. The best systems minimize documentation burden by pulling data from records that already exist, focus on a manageable number of meaningful indicators rather than tracking everything possible, and pair outcome data with process measures to give a fuller picture of what’s actually happening in care delivery.
