This line originates from the center of the C7 vertebral body, which is the seventh cervical vertebra located at the base of the neck.

The purpose of this measurement is to determine the sagittal vertical axis (SVA), which indicates the front-to-back alignment of the spine in relation to the pelvis. The line is drawn downwards from C7 to assess its position relative to the superior-posterior endplate of the S1 vertebral body, the first fused vertebra of the sacrum. In a well-aligned spine, this plumb line should ideally pass through or be within 5 millimeters of the superior-posterior endplate of S1.

Classifying Sagittal Balance

The C7 plumb line helps categorize sagittal balance into three main types: neutral, positive, and negative. Neutral balance indicates an optimal alignment where the C7 plumb line falls within 2 centimeters of the posterosuperior corner of S1.

Positive sagittal balance occurs when the C7 plumb line falls more than 2 centimeters in front of the posterosuperior corner of the S1 vertebral body. This suggests that the upper body is leaning forward relative to the pelvis. Conversely, negative sagittal balance is when the plumb line passes more than 2 centimeters behind the posterosuperior corner of S1, indicating a backward lean.

Significance of Spinal Alignment

Maintaining proper sagittal balance is significant for overall bodily function. This alignment allows the body to maintain an upright posture with minimal muscular effort and energy expenditure. When the spine’s natural curves are harmoniously aligned, mechanical stress is distributed evenly across the spinal structures.

Deviations from this balanced state, or sagittal imbalance, can lead to various problems. Individuals with sagittal imbalance may experience chronic back pain, difficulty standing upright for extended periods, and increased fatigue during daily activities. The body’s attempt to compensate for misalignment can also place abnormal pressure on spinal nerves, potentially causing symptoms like weakness or numbness.

Common Causes of Imbalance

Several factors can contribute to sagittal imbalance, altering the natural curvatures of the spine. Degenerative spinal conditions are a frequent cause, including issues like disc degeneration, spinal arthritis, and the development of excessive outward curvature in the thoracic spine (kyphosis). These conditions can lead to a flattening of the natural inward curve in the lower back, known as lumbar lordosis.

Other contributing factors include congenital conditions, where a person is born with spinal abnormalities. Spinal trauma, such as compression fractures, can also lead to changes in spinal alignment. Additionally, previous spinal surgeries, particularly certain fusion procedures, can sometimes disrupt the delicate balance of the spine, leading to sagittal imbalance.
