People may also experience burning or sharp pain, heightened sensitivity to touch, or a reduced ability to feel temperature changes.

Motor nerve damage can cause muscle weakness, cramps, or involuntary twitching. This can lead to difficulties with balance and coordination, making walking or performing fine motor tasks challenging. Muscle thinning can also occur.

Autonomic nerve damage can include digestive issues like constipation or diarrhea, changes in blood pressure leading to dizziness upon standing, or abnormal sweating. Bladder control problems and sexual dysfunction are also possible.

Diagnosis and Pathways to Care

Diagnosing neuropathy involves a comprehensive approach. A physical examination and a detailed medical history are initial steps. Neurological tests assess reflexes, muscle strength, and sensory perception.

Specific diagnostic tools help identify nerve damage and its underlying cause. Nerve conduction studies measure how quickly electrical signals travel through nerves, while electromyography assesses the electrical activity of muscles in response to nerve stimulation. Blood tests can help uncover underlying conditions such as diabetes, vitamin deficiencies, or autoimmune disorders. In some cases, a nerve biopsy or imaging tests may be necessary to pinpoint the cause and extent of nerve damage.

Treatment and Support

Managing neuropathy often involves treating the underlying cause, if identifiable, to prevent further nerve damage. For instance, individuals with diabetic neuropathy benefit from maintaining healthy blood sugar levels through diet, exercise, and medication. Addressing nutritional deficiencies or discontinuing certain medications that cause nerve damage can also be part of the treatment plan.

Symptom management is a primary aspect of care, particularly for pain. Medications such as gabapentin and duloxetine are commonly prescribed to help reduce nerve pain. Gabapentin, an anti-epileptic drug, can provide pain relief in some people with postherpetic neuralgia and diabetic peripheral neuropathy. Duloxetine, an antidepressant, has shown effectiveness in treating painful diabetic peripheral neuropathy. Amitriptyline, another antidepressant, is also used at low doses to manage nerve pain.

Non-pharmacological therapies also play a role. Physical therapy can improve muscle strength, flexibility, coordination, and balance. Occupational therapy can assist with adapting daily activities. Lifestyle adjustments, including a balanced diet and regular exercise, can support nerve health and manage symptoms.
