Is Yaz a Combination Pill? Uses and Risks

Yes, Yaz is a combination birth control pill. It contains two hormones: a synthetic estrogen (ethinyl estradiol, 0.02 mg) and a synthetic progestin (drospirenone, 3 mg). Having both an estrogen and a progestin is what makes any oral contraceptive a “combination” pill, as opposed to the progestin-only mini pill.

How Yaz Prevents Pregnancy

Like other combination pills, Yaz works through three mechanisms. Its primary action is stopping ovulation, so no egg is released for sperm to fertilize. It also thickens cervical mucus, making it harder for sperm to reach the uterus, and thins the uterine lining, which reduces the chance of implantation if an egg were somehow fertilized.

The 24/4 Dosing Schedule

Most combination pills use a 21/7 format: 21 active hormone pills followed by 7 inactive (placebo) pills. Yaz uses a 24/4 format instead, with 24 active pills and only 4 placebo pills. Those three extra days of hormones help keep levels more stable throughout your cycle, which can reduce hormone withdrawal symptoms like headaches and mood changes during the placebo days. Your period will typically be shorter with this schedule.

FDA-Approved Uses Beyond Contraception

Yaz is one of the few birth control pills approved by the FDA for more than just pregnancy prevention. It’s also approved to treat symptoms of premenstrual dysphoric disorder (PMDD), a severe form of PMS that causes significant mood disturbances. Additionally, it’s approved for moderate acne in women 14 and older who have started menstruating, though only when the patient also wants oral contraception for birth control.

These extra approvals are largely tied to drospirenone, the progestin in Yaz. Drospirenone has mild anti-androgenic properties, meaning it counteracts hormones that contribute to acne and some PMS-related symptoms. It also has a slight diuretic effect, which can reduce bloating.

How Yaz Compares to Yasmin

Yaz and Yasmin are closely related and often confused. Both contain 3 mg of drospirenone, but Yasmin has a higher dose of estrogen: 0.03 mg of ethinyl estradiol compared to Yaz’s 0.02 mg. Yasmin also follows the traditional 21/7 schedule. The lower estrogen dose in Yaz may mean fewer estrogen-related side effects like breast tenderness or nausea for some people, though individual responses vary.

Blood Clot Risk With Drospirenone

All combination birth control pills carry a small risk of blood clots, but drospirenone-containing pills like Yaz carry a slightly higher risk than older formulations. The FDA estimates that about 10 in 10,000 women taking a drospirenone pill will develop a blood clot over one year, compared to roughly 6 in 10,000 women on older combination pills. For context, both numbers are quite low in absolute terms, and the risk of blood clots during pregnancy is considerably higher than either.

This risk is most relevant if you smoke (especially over age 35), have a personal or family history of blood clots, or have other cardiovascular risk factors. These are the same considerations that apply to all combination pills, just slightly amplified with drospirenone.

Potassium and Kidney Considerations

One thing that sets Yaz apart from many other combination pills is drospirenone’s effect on potassium levels. Drospirenone can raise potassium in the blood, which is usually harmless but becomes a concern if you have kidney disease, liver disease, or adrenal insufficiency. Yaz is contraindicated for people with these conditions.

If you take certain medications daily, your potassium levels may need monitoring during the first month on Yaz. This includes common anti-inflammatory drugs like ibuprofen or naproxen (when used long-term, not occasionally), potassium-sparing diuretics like spironolactone, ACE inhibitors, and potassium supplements. High potassium can affect heart rhythm, which is why this interaction matters.

Generic Versions

Yaz is available in generic form, which contains the same 0.02 mg ethinyl estradiol and 3 mg drospirenone in the same 24/4 format. Generics are typically significantly cheaper. If your pharmacy substitutes a generic, the active ingredients and dosing schedule will be identical to brand-name Yaz.
