In the United States, Viagra is prescription only. You cannot buy it over the counter at a pharmacy, and no application to change that status has been approved by the FDA. The UK is a different story, where a lower-dose version called Viagra Connect has been available without a prescription since 2018. Your options depend on where you live.
Prescription Rules in the U.S.
The FDA classifies Viagra (sildenafil citrate) as a prescription medication for erectile dysfunction. That means a licensed provider must evaluate you and write a prescription before a pharmacist can dispense it. This applies to both brand-name Viagra and generic sildenafil, which contain the same active ingredient.
The good news is that getting a prescription has become significantly easier. Most telehealth platforms now offer consultations specifically for ED treatment. About 93% of these direct-to-consumer platforms start with an online questionnaire rather than a video call. Depending on your answers and your state’s laws, a prescriber may approve the medication without a live conversation, though some states require real-time contact by phone or video. The process typically takes under two business days. One limitation of this approach: these platforms rely on self-reported blood pressure and health history, which means certain cardiovascular risks could be missed without an in-person exam.
Over-the-Counter Access in the UK
The UK’s medicines regulator approved Viagra Connect for sale without a prescription in 2018. It’s available in pharmacies after a brief consultation with the pharmacist, who screens for contraindications. The key difference from prescription Viagra is dosage: Viagra Connect comes in a single 50 mg strength only, while the prescription version is available in 25 mg, 50 mg, and 100 mg tablets.
The pharmacist consultation isn’t just a formality. You’ll be asked about heart conditions, medications you take (especially nitrates and certain antiviral drugs), and liver or kidney problems. People with severe kidney impairment or liver disease may need a lower 25 mg dose, which requires a prescription. If Viagra Connect doesn’t work after several attempts, you’re advised to see a doctor rather than simply increasing how often you take it. The maximum is one tablet per day, taken about an hour before sexual activity.
Why a Prescription Exists for This Drug
Sildenafil lowers blood pressure as part of how it works. On its own, this drop is mild and temporary for most men. But combined with nitrate medications, commonly prescribed for chest pain (angina), the effect becomes dangerous. In clinical testing, men who took sildenafil alongside a common nitrate medication experienced blood pressure drops roughly double what the nitrate alone caused, with systolic pressure falling by an average of 52 points when standing. That kind of drop can cause fainting, heart attack, or stroke.
Nitrates aren’t the only concern. Certain HIV medications, some antifungal drugs, and alpha-blockers (used for enlarged prostate or high blood pressure) all interact with sildenafil in ways that require dose adjustments. Men with conditions like sickle cell anemia that increase the risk of prolonged erections also need medical guidance. A prescription requirement ensures someone with clinical training reviews these risks before you take the drug.
Generic Sildenafil vs. Brand-Name Viagra
Generic sildenafil has been available in the U.S. since December 2017, and the price difference is staggering. A typical 50 mg dose of generic sildenafil costs as little as 44 cents per pill when purchased in quantities of 30. Brand-name Viagra for the same dose runs over $67 per pill. Both require a prescription, and both contain identical active ingredients at identical strengths (25 mg, 50 mg, or 100 mg). From a practical standpoint, there is almost no reason to pay for the brand name unless your insurance specifically covers it at a lower copay, which is rare.
The price collapse of the generic has been dramatic. Between late 2017 and mid-2021, the list price for six tablets of 100 mg generic sildenafil fell by 98%, from $265 to $4. Over that same period, the equivalent brand-name Viagra actually increased in price by 27%.
The Risk of Buying Without a Prescription Online
The prescription requirement drives some men to buy from unregulated online sellers, and the risks here are serious. Estimates suggest that anywhere from 44% to 90% of medications sold over the internet are counterfeit. The World Health Organization has warned that medicines purchased from websites that hide their physical address are counterfeit more than half the time.
What’s inside these counterfeits varies wildly. When the Dutch National Institute for Public Health analyzed 370 samples of illegally sold Viagra, only 10 were genuine. A broader analysis found that just 10.1% of counterfeit samples contained the active ingredient within 10% of what the label claimed. Some pills had far less sildenafil than advertised. Others contained no sildenafil at all.
The substituted ingredients are genuinely alarming. Counterfeit Viagra seized across Europe has been found to contain amphetamine, commercial paint, printer ink, the antidepressant fluoxetine, the antibiotic chloramphenicol, and various other unrelated pharmaceuticals. One batch from Hungary contained nothing but amphetamine. Another seized in the UK had caffeine and lactose filler with only 30 mg of sildenafil instead of the labeled 100 mg. Taking a pill that might contain an unpredictable dose of the active ingredient, mixed with unknown contaminants, alongside whatever other medications you take is a genuinely dangerous gamble.
How to Get a Prescription Easily
If you’re in the U.S., your most straightforward options are your primary care doctor, a urologist, or a telehealth platform. The telehealth route is the fastest and most private for many men. You’ll fill out a health questionnaire covering your medical history, current medications, blood pressure, and heart health. If nothing in your profile raises a red flag, a prescription can be approved within a day or two and sent directly to a pharmacy or the platform’s own fulfillment service.
If you’re in the UK, you can walk into most pharmacies and purchase Viagra Connect 50 mg after the pharmacist consultation, no appointment needed. For higher or lower doses, you’ll still need a prescription from a GP or online prescriber. Several other countries, including New Zealand and Poland, have also approved some form of pharmacist-dispensed sildenafil, though the specific rules vary.
