Both salt water, commonly known as saline solution, and hydrogen peroxide are household items frequently used for cleansing and minor disinfection. Hydrogen Peroxide: Mechanism and Drawbacks

Hydrogen peroxide (H2O2) functions as a powerful oxidizing agent. When the common 3% solution contacts tissue, an enzyme called catalase causes it to rapidly break down into water and oxygen gas. This rapid release of oxygen causes the characteristic foaming action, which mechanically helps to lift and remove debris, dead tissue, and bacteria from a surface.

The strong oxidative action, while effective at killing germs, is not specific; it also damages healthy human cells. This destructive effect is known as cytotoxicity, and it impairs the function of cells necessary for tissue repair, such as fibroblasts. Hydrogen peroxide’s cytotoxicity can harm the cells needed for closure, actively slowing the repair of the injury.

Oral and Throat Care

When gargling for sore throats or mouth irritations, a warm saline rinse is the safest and most effective home remedy. The salt helps draw fluid from swollen tissues, soothing discomfort and reducing inflammation with minimal risk of irritation. Diluted hydrogen peroxide can be used occasionally as a powerful disinfectant for conditions like canker sores, but it is less suitable for routine use due to the risk of irritating the soft tissues of the mouth.

Nasal Irrigation

Only saline is appropriate for nasal and sinus irrigation. The delicate ciliated cells lining the nasal passages are highly sensitive, and the harsh oxidative properties of hydrogen peroxide can damage them, potentially worsening the condition. In the specialized case of deep or heavily contaminated wounds, hydrogen peroxide may play a limited role as an aggressive initial flush to remove significant debris and necrotic tissue.
