That simple routine is the foundation, but the details around timing, food interactions, and other medications matter more than most people realize. Small changes in how you take this pill can shift how much your body actually absorbs, potentially throwing off your thyroid levels.

Why an Empty Stomach Matters

Levothyroxine is a finicky medication. Food in your stomach interferes with absorption, and the difference isn’t trivial. One crossover study found that taking the pill within 20 minutes of breakfast raised TSH levels nearly three times higher than taking it on an empty stomach (2.93 vs. 1.06 mIU/L), meaning significantly less medication was getting into the bloodstream. The standard recommendation is to take it first thing in the morning, at least 30 to 60 minutes before your first meal.

Swallow it with a full glass of plain water. A sip or two before the pill helps it go down, then finish the rest of the glass. This isn’t just about comfort. If the tablet lodges in your esophagus, it can cause irritation or inflammation of the tissue lining your throat. Water moves it quickly into the stomach where it can dissolve properly.

Coffee Changes the Equation

If your morning routine involves reaching for coffee right after your pill, that’s a problem. Research has shown that coffee, particularly espresso, can decrease levothyroxine absorption. The mechanism likely involves coffee speeding up gut motility and changing the stomach environment before the medication has fully dissolved. The safest approach is to treat coffee like food: wait at least 60 minutes after taking your pill before that first cup.

Morning vs. Bedtime Dosing

Most people take levothyroxine in the morning, but bedtime dosing is a legitimate alternative if your morning schedule makes the fasting window impractical. The key requirement is the same: an empty stomach, meaning at least two hours after your last meal.

The research on which timing produces better thyroid levels is mixed. A 2010 double-blind crossover study of 90 adults found that bedtime dosing actually lowered TSH by 1.25 mIU/L compared to morning dosing, suggesting better absorption. But a 2009 crossover study of 65 patients found the opposite: bedtime dosing produced higher TSH levels than fasting morning doses (2.19 vs. 1.06 mIU/L). A 2011 study of newly diagnosed patients found no significant difference between the two approaches at 12 weeks.

The inconsistency likely comes down to how strictly people followed the fasting rules. If you genuinely stop eating two to three hours before bed and take the pill right before sleep, bedtime dosing can work well. If you tend to snack late, morning dosing with a proper fasting window is more reliable.

Supplements That Block Absorption

Calcium and iron are the two biggest culprits. Both bind to levothyroxine in the gut, forming compounds your body can’t absorb. The recommended gap is at least four hours between your thyroid pill and any product containing calcium or iron. That includes multivitamins, antacids, and fortified foods.

Fiber also reduces absorption. If you eat a high-fiber diet, you’re not doing anything wrong, but your doctor may need to adjust your dose to compensate. Soy products present a similar challenge. If soy milk, tofu, or edamame are regular parts of your diet, wait at least an hour after taking levothyroxine before consuming them.

Acid-Reducing Medications

Proton pump inhibitors, the type of medication commonly used for acid reflux, reduce stomach acid. Levothyroxine tablets need an acidic environment to dissolve fully, so less acid means less of the drug gets absorbed. If you take both medications, your doctor may need to increase your levothyroxine dose or switch you to a liquid formulation, which studies have shown is not affected by reduced stomach acid in the same way.

Stick With One Manufacturer
