Switching from Saxenda to Wegovy is straightforward: you take your last Saxenda injection one day, then start Wegovy the next. No washout period is needed between the two medications. Even though both drugs work through the same pathway (GLP-1 receptor agonists), the transition does involve changes in dosing, injection frequency, and what you can expect in terms of results.
Why People Switch
The most common reason is better weight loss results. In the STEP 8 clinical trial, which directly compared the two drugs head-to-head in 338 participants, Wegovy produced an average body weight reduction of 15.8% over 68 weeks, compared to 6.4% with Saxenda. That’s roughly two and a half times the weight loss. For someone who has plateaued on Saxenda or wants stronger results, Wegovy offers a meaningful step up.
Convenience is another factor. Saxenda requires a daily injection, while Wegovy is once weekly. For people tired of injecting every day, that shift alone can make a big difference in quality of life and adherence.
Starting Dose and Titration
Even if you’ve been on the maximum 3.0 mg dose of Saxenda, you won’t jump straight to a high dose of Wegovy. The VA’s conversion guidance recommends starting Wegovy at 0.5 mg once weekly for four weeks. This is actually the second step on Wegovy’s standard titration ladder, which normally begins at 0.25 mg. Starting at 0.5 mg reflects the fact that your body already has experience with a GLP-1 medication and can tolerate a slightly higher entry point.
From there, your prescriber will titrate you up through the remaining doses (1.0 mg, then 1.7 mg, then the maintenance dose of 2.4 mg), typically spending four weeks at each level. The full ramp-up takes several months, so patience is important. Rushing the titration increases the risk of nausea and other GI side effects.
Timing the Switch
The timing is simple. Take your final Saxenda injection as you normally would, then start Wegovy the following day. Saxenda clears your system relatively quickly since it’s designed for daily dosing, so there’s no need to wait days or weeks between medications. Your prescriber will help you pick a day of the week for your Wegovy injection that works with your schedule, since you’ll be sticking with that same day each week going forward.
What Changes Day to Day
The biggest practical shift is going from a daily routine to a weekly one. With Saxenda, you inject at roughly the same time each day. With Wegovy, you pick one day per week and can inject at any time that day, with or without food.
Storage is similar for both medications. Unused Wegovy pens need refrigeration, just like Saxenda. Saxenda pens must be kept between 36°F and 46°F before first use, and once opened, they can be stored at room temperature (below 86°F) or in the fridge for up to 30 days. Wegovy pens come as single-use prefilled injectors, so there’s no multi-dose pen to track or store after opening. Each week you use a fresh pen.
Both medications should be disposed of in an FDA-approved sharps container. If you already have one from Saxenda, you can keep using it for Wegovy.
Side Effects During the Transition
Because Saxenda and Wegovy target the same receptor, the side effect profile is familiar: nausea, vomiting, diarrhea, constipation, and stomach pain are the most common issues. Having tolerated Saxenda doesn’t guarantee a smooth ride on Wegovy, though. Semaglutide is a more potent drug, and each dose increase during titration can bring a fresh wave of GI symptoms.
Most people find that side effects peak in the first few days after a dose increase and then settle over the following weeks. Eating smaller meals, avoiding greasy or heavy foods, and staying hydrated can help. If nausea is severe or persistent at a particular dose, your prescriber may keep you at that level for an extra four weeks before moving up.
What to Expect for Weight Loss
If you switched because your weight loss stalled on Saxenda, Wegovy’s stronger effect gives you a realistic chance of breaking through that plateau. The STEP 8 trial showed participants on semaglutide lost an additional 9.4 percentage points of body weight compared to those on liraglutide over the same time period. That’s a clinically significant gap.
Keep in mind that those trial results reflect people starting from scratch on each medication, not switching mid-treatment. Your individual results will depend on where you are in your weight loss journey, your starting weight, diet, activity level, and how well you tolerate the maintenance dose. Still, the data strongly suggests that Wegovy delivers more weight loss for most people, which is why the switch is so common.
