What is Liver Stiffness?

If fibrosis continues unchecked, it can advance to cirrhosis, a severe condition where extensive scar tissue replaces healthy liver cells, leading to impaired liver function. Cirrhosis can eventually result in liver failure or liver cancer. Liver stiffness is quantified in kilopascals (kPa) using non-invasive imaging techniques like transient elastography.

Common factors that contribute to liver stiffness include chronic viral hepatitis (Hepatitis B and C), non-alcoholic fatty liver disease (NAFLD), and alcoholic liver disease. Autoimmune conditions targeting the liver and certain genetic disorders can also lead to increased stiffness. Early detection of liver stiffness is important because interventions can potentially halt or even reverse early stages of fibrosis.

Lifestyle and Dietary Strategies

Adopting specific lifestyle and dietary changes can significantly influence liver health and potentially reduce stiffness. A balanced, liver-friendly diet emphasizes whole, unprocessed foods. The Mediterranean diet, for example, is rich in fruits, vegetables, whole grains, nuts, and olive oil, and limits red meat and processed foods, which can positively impact liver health by reducing fat and inflammation.

Weight management is also important, particularly for individuals with non-alcoholic fatty liver disease (NAFLD). Losing even 3% to 5% of body weight can offer benefits, and a 7% to 10% weight reduction is often recommended to improve liver fat content, inflammation, and fibrosis.

Avoiding or significantly reducing alcohol consumption is another important step, as alcohol is a major contributor to liver damage and stiffness. Studies show that liver stiffness can rapidly decline in heavy drinkers after just seven days of abstinence, with further reductions seen over several weeks. This suggests that non-fibrotic lesions, such as alcoholic hepatitis, also influence liver stiffness.

Regular physical activity also benefits liver health by improving blood flow, decreasing liver inflammation, and reducing liver fat, even independent of significant weight loss. Aim for at least 150 minutes of moderate-intensity aerobic exercise, such as brisk walking, or 75 minutes of vigorous-intensity exercise per week. Resistance training two days a week can also be beneficial. Other methods include magnetic resonance elastography (MRE), which combines MRI with sound waves to map liver stiffness, and various blood tests that can indicate liver function and inflammation.

These tests help estimate the degree of liver damage, track disease progression or regression, and guide treatment decisions. While FibroScan is effective for assessing scarring, it does not replace conventional ultrasound, which is used to evaluate the liver’s structure and screen for complications like hepatocellular carcinoma.
