How to Protect Your Teeth From Acid Reflux

Gastroesophageal Reflux Disease (GERD), or acid reflux, occurs when stomach contents flow back into the esophagus and sometimes reach the mouth. This involuntary regurgitation exposes teeth to highly corrosive stomach acids, making GERD a common cause of severe dental erosion. When this acid contacts the tooth surface, it chemically dissolves the protective outer layer of enamel. Taking proactive measures is necessary to mitigate the destructive effects of stomach acid on your smile.

How Acid Reflux Damages Tooth Enamel

Tooth enamel is the hardest substance in the human body, composed primarily of calcium phosphate crystals. While designed to withstand chewing, enamel is highly vulnerable to acid. Enamel begins to dissolve, a process called erosion, when the pH level drops below 5.5. Stomach acid is dramatically more acidic, typically ranging between pH 2.0 and 3.0. When this gastric fluid washes over the teeth, it rapidly strips away the mineral components. This chemical dissolution process weakens the enamel, leading to visible loss of tooth structure, increased sensitivity, and a higher risk of damage.

Immediate Actions Following an Acid Event

After an acid reflux episode or vomiting, the natural reaction is often to brush immediately, but this causes further harm. Stomach acid softens the enamel, placing it in its most vulnerable state. Brushing at this point scrubs away the softened outer layer, accelerating erosion.

Instead of brushing, immediately neutralize the acid and rinse it away. Swish thoroughly with plain water to dilute the corrosive fluids. A more effective step is rinsing with an alkaline solution, such as one teaspoon of baking soda dissolved in a cup of water, to neutralize the low pH.

Milk also works well for rinsing, as its calcium content and neutral pH help buffer the acid. After rinsing, wait at least 30 minutes before brushing. This delay allows saliva time to restore the mouth’s natural pH and lets the enamel reharden.

Modifying Your Daily Dental Routine

Long-term protection from acid erosion requires specific adjustments to your daily hygiene routine and professional care. Fluoride is a primary component in this defense strategy because it strengthens the enamel structure, making it more resistant to acid attacks. Patients with acid erosion should use a high-fluoride toothpaste to encourage the remineralization of damaged surfaces. A dentist may prescribe a high-concentration fluoride product, such as a 5000 ppm fluoride paste, for maximum reinforcement against chemical wear.

When selecting a toothbrush, opt for a soft-bristled version to minimize abrasion on the already weakened enamel. Choosing a low-abrasive toothpaste also reduces physical wear when brushing.

Chewing sugar-free gum after meals stimulates saliva flow. Saliva is the mouth’s natural defense, rich in minerals and bicarbonates that neutralize acid and help repair the enamel. Regular dental checkups are necessary to monitor erosion and discuss protective treatments. For significant damage, a dentist may recommend interventions like dental bonding or sealants to cover vulnerable tooth surfaces.

Controlling Acid Reflux Through Lifestyle

The most direct way to protect the teeth is to reduce the frequency and severity of the acid reaching the mouth. Manage reflux by identifying and avoiding common trigger foods that relax the lower esophageal sphincter or increase acid production. These often include spicy foods, fatty or fried meals, chocolate, peppermint, and acidic items like citrus fruits and tomatoes.

Meal timing plays a significant role in preventing nocturnal reflux, which is particularly damaging because saliva production decreases during sleep. Finish your last meal or snack at least two to three hours before lying down. When sleeping, elevate the head of your bed by six to eight inches using blocks or a specialized wedge. This ensures stomach contents are less likely to flow back into the esophagus and reach the oral cavity.
