Giving yourself an Ozempic shot is straightforward once you know the steps. The pen comes pre-filled and uses a small, thin needle that goes just under the skin, making the process quick and relatively painless. You inject it once a week, on the same day each week, in one of three body areas.
Check Your Pen Before Each Use
Before attaching a needle, look at the liquid inside the pen window. It should be clear and colorless. If it looks cloudy, has particles floating in it, or has changed color, don’t use that pen. Also confirm the label says Ozempic and check the expiration date.
Attach a New Needle
Every injection requires a fresh needle. Tear off the paper tab on the needle package, then push the needle straight onto the pen tip and twist it on until it’s snug. The needle comes covered by two caps: an outer cap and an inner cap. Pull both off before injecting. Save the outer cap, though. You’ll need it to safely remove the needle after your shot.
Do a Flow Check on a New Pen
If this is the first time you’re using a brand-new pen, you need to prime it before your first dose. This clears any air from the needle and confirms the pen is working. Turn the dose selector to the flow check symbol, hold the pen with the needle pointing up, and press the dose button. A small drop of liquid should appear at the needle tip. If nothing comes out, repeat the process up to six times. If you still don’t see a drop, the needle may be blocked. Replace it and try again.
You only need to do this flow check once per new pen, not before every weekly injection.
Select Your Dose
Turn the dose selector until it shows your prescribed dose. Ozempic follows a gradual schedule: you start at 0.25 mg once a week for the first four weeks. This starting dose is meant to let your body adjust, not to provide the full treatment effect. After those four weeks, the dose increases to 0.5 mg weekly. If your prescriber decides you need more, the dose can go up to 1 mg after at least another four weeks, and eventually to a maximum of 2 mg weekly. Each step up requires a minimum of four weeks at the current dose.
The pen clicks as you turn the selector, and the dose window displays the exact amount. If you dial past your dose, you can turn it back without wasting any medication.
Choose and Rotate Your Injection Site
You have three options for where to inject: your lower abdomen (at least two inches from your belly button), the front of your thigh, or the back of your upper arm. All three locations deliver the medication with equal effectiveness.
The key habit to build is rotating between these sites. If you inject the same spot repeatedly, you’re more likely to develop skin irritation, small lumps, or changes in the fatty tissue under the skin. A simple approach is picking a different area each week. Some people find certain spots more comfortable than others, and that’s fine. Just don’t use the exact same patch of skin week after week. If you inject in the same general area (say, your abdomen two weeks in a row), shift at least an inch or two from where you injected last time.
How to Inject
Clean the skin at your chosen site with an alcohol swab and let it dry. Pinch a fold of skin gently with one hand. With the other, insert the needle straight into the skin fold at a 90-degree angle. Press the dose button all the way in with your thumb and hold it there.
Here’s the part people often rush: after the dose counter returns to zero, keep the needle in your skin and hold the button down for a slow count of six seconds. This ensures the full dose has been delivered. If you pull out too quickly, you may see liquid leaking from the needle tip, which means you didn’t get your complete dose. After the six seconds, withdraw the needle and release the skin fold.
A small drop of blood at the injection site is normal and nothing to worry about. You can press a cotton ball or gauze lightly over the spot, but don’t rub it.
Remove and Dispose of the Needle
Carefully guide the outer needle cap back onto the needle (without using your fingers near the tip), then unscrew the capped needle from the pen. Place the used needle directly into a sharps disposal container. A rigid, puncture-resistant container with a tight lid works. You can buy FDA-cleared sharps containers at most pharmacies, or use a heavy-duty plastic container like a laundry detergent bottle in a pinch.
Never toss loose needles into household trash or recycling. When your sharps container is about three-quarters full, seal it and dispose of it through your community’s guidelines. Options vary by location but commonly include drop-off sites at pharmacies, hospitals, or fire stations, as well as mail-back programs. Replace the pen cap and store it until your next weekly dose. Never store the pen with a needle attached, as this can cause air bubbles or leaking.
Storing Your Pen
An unused Ozempic pen belongs in the refrigerator, between 36°F and 46°F. Once you’ve used the pen for the first time, you have 56 days before it must be discarded, regardless of how much medication is left inside. During those 56 days, you can keep it in the refrigerator or store it at room temperature (59°F to 86°F). Many people find it easier to keep the pen in the fridge door, which stays within the right range and makes it simple to remember.
Don’t freeze Ozempic, and don’t leave it in a hot car or in direct sunlight. If a pen has been exposed to temperatures outside the recommended range, it’s safest to discard it.
Timing Your Weekly Shot
Pick a day of the week that’s easy to remember and stick with it. You can inject at any time of day, with or without food. If you need to shift your injection day, you can do so as long as at least two days (48 hours) have passed since your last dose. Setting a recurring phone alarm for the same day and time each week helps build the routine.
If you forget a dose and it’s been fewer than five days since you were supposed to inject, take it as soon as you remember. If more than five days have passed, skip that dose and take the next one on your regular day.
