How to Get Sildenafil: In-Person and Telehealth Options

Sildenafil is a prescription medication in the United States, meaning you need a licensed healthcare provider to authorize it before a pharmacy can dispense it. The good news is that getting a prescription has become faster and more affordable than ever, with multiple pathways ranging from an in-person doctor visit to a five-minute online questionnaire. Generic sildenafil can cost under a dollar per dose, a fraction of what brand-name Viagra runs.

Why You Need a Prescription

The FDA classifies sildenafil as a prescription drug because it carries real risks for certain people. The most serious interaction is with nitrate medications, commonly prescribed for chest pain and heart conditions. Combining sildenafil with nitrates can drop blood pressure to dangerous, even fatal, levels. People with active coronary problems, congestive heart failure with low blood pressure, severe kidney or liver impairment, or those on complex blood pressure regimens also need careful evaluation before taking it.

These aren’t theoretical risks. A prescriber screens for them because the consequences of missing one can be severe. That screening is what separates a legitimate prescription from buying pills off an unverified website.

Getting a Prescription In Person

The most straightforward route is scheduling an appointment with your primary care doctor or a urologist. The visit is typically brief. Your doctor will ask about your symptoms, review your medications (especially nitrates and certain antibiotics or antacids that affect how your body processes sildenafil), check your blood pressure, and possibly order basic bloodwork. If everything checks out, you’ll leave with a prescription you can fill at any pharmacy.

Many men feel awkward bringing up erectile dysfunction, but doctors handle these conversations routinely. ED affects roughly half of men over 40 to some degree, so your provider won’t be surprised.

Using a Telehealth Platform

Online telehealth services have become the most popular way to get sildenafil, largely because the process is fast and doesn’t require an in-person exam. A study of direct-to-consumer ED platforms found that 93% rely on online intake forms rather than face-to-face consultations. Most advertise prescriber review within two business days, and many deliver the medication directly to your door.

Here’s what the process typically looks like:

  • Fill out a health questionnaire. You’ll answer questions about your medical history, current medications, blood pressure, and symptoms.
  • A prescriber reviews your answers. At some platforms this is always a physician; at others it may be a nurse practitioner or physician assistant. About half of platforms only initiate direct contact with the prescriber if your answers raise concerns or if your state’s laws require it.
  • Receive your prescription. If approved, the platform either sends the prescription to your preferred pharmacy or ships the medication from their own.

One limitation worth knowing: these platforms rely on self-reported vital signs. If you haven’t had your blood pressure checked recently or you have undiagnosed heart issues, an online form may not catch problems that an in-person visit would. If you have any cardiovascular history, an in-person evaluation is the safer first step.

What Dosage to Expect

Sildenafil for ED comes in 25 mg, 50 mg, and 100 mg tablets. Most prescribers start at 50 mg, taken about an hour before sexual activity. Adults 65 and older typically start at 25 mg. Your doctor can adjust the dose up to 100 mg or down to 25 mg based on how well it works and whether you experience side effects. The firm rule is no more than one dose per day.

You may also see sildenafil available in 20 mg tablets. That strength is technically approved for a lung condition called pulmonary arterial hypertension (sold under the brand name Revatio), but doctors sometimes prescribe the 20 mg tablets off-label for ED because they can be cheaper. In that case, your doctor might tell you to take two or three tablets to reach your target dose.

Generic Sildenafil vs. Brand-Name Viagra

Generic sildenafil and Viagra contain the identical active ingredient and work the same way. The price difference, however, is staggering. Generic sildenafil runs as low as 44 cents per 50 mg dose when purchased in quantities of 30. Brand-name Viagra for the same dose costs upward of $67 per pill. Six tablets of 100 mg generic sildenafil can be found for under $10 at some pharmacies with a discount coupon, while the equivalent Viagra runs over $400.

Generics became available in the U.S. in late 2017, and prices dropped 98% between then and mid-2021. There is no clinical reason to pay for the brand name. Ask your prescriber for generic sildenafil, or ask your pharmacist about substitution if a prescription is written for Viagra.

How It Works Once You Take It

Sildenafil doesn’t cause an automatic erection. It works by increasing blood flow to the penis when you’re sexually aroused. Most men can achieve a usable erection within 30 minutes of taking a dose, and some respond in as little as 12 minutes. Peak blood levels hit around the one-hour mark. The medication remains active for at least four hours, with a half-life of three to five hours.

Taking it on an empty stomach speeds absorption. A heavy or high-fat meal can delay the onset noticeably.

Common Side Effects

About a third of users experience at least one side effect. In clinical practice, the most frequently reported are flushing (31%), headache (25%), nasal congestion (19%), and heartburn (11%). These tend to be mild and short-lived. Higher doses are linked to a greater chance of side effects, which is one reason prescribers start at the lowest effective dose and adjust from there.

Less common effects include temporary changes in color vision (a slight blue tint), dizziness, and light sensitivity. Rare but serious reactions like sudden vision loss or an erection lasting more than four hours require immediate medical attention.

Avoiding Counterfeit Products

Sildenafil is one of the most counterfeited medications in the world. Products sold without a prescription through unverified websites, gas stations, or “herbal” supplements labeled with names like “U.S.A Viagra” frequently contain unknown quantities of active ingredients, the wrong ingredients entirely, or dangerous contaminants. The FDA has issued numerous warnings about supplements found to contain hidden sildenafil or similar compounds at unpredictable doses.

To protect yourself, fill prescriptions only through state-licensed pharmacies, whether brick-and-mortar or online. Verified online pharmacies will always require a prescription, display a valid pharmacy license, and have a licensed pharmacist available for questions. If a site sells sildenafil without asking about your health history, it is not a legitimate source.

Sildenafil Without a Prescription Outside the U.S.

In the United Kingdom, sildenafil is available over the counter at pharmacies under the brand name Viagra Connect (50 mg). You don’t need a doctor’s appointment, but the pharmacist conducts a health screening before selling it. They’ll ask about your medical history, current medications, and general health. If the pharmacist has any concerns about whether sildenafil is safe for you, they’ll refer you to a doctor instead. This model doesn’t exist in the U.S., where all strengths of sildenafil remain prescription-only.
