This allows for a more direct calculation, leveraging the exact age of the embryo at the time of transfer.

Calculating Due Date Based on Transfer Day

The due date for an IVF pregnancy is primarily calculated by adding a specific number of days to the embryo transfer date, accounting for the embryo’s developmental stage. For a Day 5 embryo transfer, typically a blastocyst, the estimated due date is calculated by adding 261 days to the transfer date. This method considers the embryo to be approximately 5 weeks and 2 days gestational age at the time of transfer.

For a Day 3 embryo transfer, representing a cleavage-stage embryo, the due date is determined by adding 263 days to the transfer date. This means the embryo is considered to be about 5 weeks gestational age at transfer. These calculations apply to both fresh and frozen embryo transfers (FET), as the key factor is the embryo’s age when transferred.

Another way to conceptualize the calculation is to recognize that a full-term pregnancy is generally 266 days (38 weeks) from conception. For a Day 5 transfer, you subtract 5 days from 266, resulting in 261 days to add to the transfer date. For a Day 3 transfer, you subtract 3 days from 266, meaning 263 days are added to the transfer date.

The Role of Ultrasound Confirmation

While IVF provides a precise initial due date, early ultrasound confirmation plays an important role in verifying and sometimes fine-tuning this estimate. Ultrasounds performed between 6 and 13 weeks of gestation are highly accurate for dating a pregnancy, particularly when measuring the crown-rump length (CRL) of the embryo or fetus. The CRL, which is the measurement from the top of the head to the bottom of the torso, is considered the most accurate parameter for dating during this early period because there is minimal biological variability in fetal growth rates.

Early first-trimester ultrasounds can predict the due date with an accuracy of approximately +/- 5 to 7 days. Even with IVF pregnancies, where the conception date is known, an early ultrasound provides a visual confirmation of growth and development, which can sometimes lead to minor adjustments of the initial calculated due date.

Understanding Due Date Flexibility and Accuracy

Even with the precision afforded by IVF and early ultrasound, a due date remains an estimate rather than a fixed delivery date. Only a small percentage, approximately 4% to 5%, of babies are born on their exact due date. Most deliveries occur within a two-week window surrounding the estimated date.

Individual variations in fetal growth rates contribute to this flexibility, as babies do not all develop at precisely the same pace. The due date serves as a guide for monitoring pregnancy progression and for planning medical care.
