How to Become a Nurse Aide: Steps, Exam & Pay

Becoming a nurse aide is one of the fastest paths into healthcare. Most people can go from zero experience to certified and job-ready in as little as four to eleven weeks, after completing a state-approved training program and passing a competency exam. Here’s what each step looks like.

Basic Eligibility

Federal regulations don’t set a specific age or education requirement for nurse aide training programs, but most states require you to be at least 16 or 18 years old. Many programs ask for a high school diploma or GED, though this isn’t universal. Some states let you enroll without one.

Before you start clinical training in a facility, you’ll need a criminal background check. Long-term care facilities are required to run both state and national criminal history checks, including FBI fingerprint screening, on all direct patient access employees. You’ll submit a set of 10 rolled fingerprints and a written disclosure of any disqualifying offenses. Programs also commonly require a current TB test, drug screening, and up-to-date immunizations, though these requirements vary by state and training site.

What Training Involves

Federal law requires nurse aide training programs to include at least 75 clock hours of instruction. Of those, a minimum of 16 hours must be supervised practical training, where you practice hands-on skills under the direct supervision of a registered nurse or licensed practical nurse. The remaining 59 hours are classroom instruction.

Before you’re allowed any direct contact with a resident, you must complete at least 16 hours covering five foundational areas: communication and interpersonal skills, infection control, safety and emergency procedures (including the Heimlich maneuver), promoting residents’ independence, and respecting residents’ rights. Only after demonstrating proficiency in these areas can you begin working with actual patients during clinicals.

Many states exceed the 75-hour federal minimum. California, for example, requires 160 hours. New York has extended the federal minimum but structures its curriculum around that same 75-hour framework. Check your state’s specific requirements before enrolling, because the program you choose must be approved by your state health department to count toward certification.

All training must be conducted by or under the supervision of a registered nurse with at least two years of nursing experience, including at least one year in long-term care.

Where to Find Programs

Nurse aide training programs are offered through community colleges, vocational schools, the American Red Cross, and many nursing homes and hospitals. Some long-term care facilities offer free training in exchange for a commitment to work there after certification. This is worth asking about, since it eliminates tuition costs entirely.

Program costs typically range from about $500 to $2,000 depending on your location and the institution. As a reference point, one New Jersey program charges $1,500 total, broken down into $1,375 for tuition, $50 in fees, and $75 for books and materials. Community college programs tend to sit at the lower end. Private vocational schools can charge more. Budget for the background check, fingerprinting, and any required medical screenings on top of tuition.

Passing the Competency Exam

After finishing your training program, you’ll take a state competency evaluation. This exam has two parts: a written (or oral) knowledge test and a clinical skills demonstration. During the skills portion, you’ll be asked to perform several randomly selected tasks in front of an evaluator, such as taking vital signs, positioning a patient in bed, or assisting with personal hygiene.

Most states give you three attempts to pass. If you fail after three tries, you’ll typically need to retake the training program before testing again. Once you pass, your name is added to your state’s nurse aide registry, and you can begin working as a Certified Nursing Assistant (CNA).

What the Job Looks Like

Nurse aides provide the most direct, hands-on patient care in healthcare settings. Your daily work centers on helping patients with activities of daily living: bathing, dressing, eating, toileting, and moving around safely. You’ll also take and record vital signs like blood pressure, pulse, and temperature, and report any changes in a patient’s condition to the nursing staff.

Most CNAs work in nursing homes and long-term care facilities, though hospitals, assisted living centers, home health agencies, and rehabilitation clinics also hire them. The work is physically demanding. You’ll spend long stretches on your feet, and lifting or repositioning patients is routine. Shifts often include evenings, weekends, and holidays.

Pay and Job Prospects

The median annual wage for nursing assistants was $39,530 as of May 2024, according to the Bureau of Labor Statistics. Pay varies significantly by state, facility type, and shift differential. Working night or weekend shifts often comes with a pay bump.

Employment is projected to grow 2 percent from 2024 to 2034, which is slower than average. That said, high turnover in the field means there are consistently open positions. Demand remains steady in long-term care as the population ages.

Keeping Your Certification Active

CNA certification isn’t permanent. You’ll need to renew it, and the specifics depend on your state. Michigan, for example, requires 12 hours of continuing education per year, totaling 24 hours over a two-year renewal cycle. Most states also require proof that you’ve worked a minimum number of paid hours as a nurse aide during the renewal period. If your certification lapses, you may need to retake the competency exam or, in some cases, complete a new training program.

Using CNA as a Stepping Stone

Many nurses, physician assistants, and other healthcare professionals started as nurse aides. Working as a CNA gives you direct patient care experience that strengthens applications to nursing school and other clinical programs. Some employers offer tuition reimbursement for CNAs pursuing further education, making it a practical way to build both experience and financial support before advancing your career.
