Becoming a certified nursing assistant (CNA) is one of the fastest paths into healthcare, typically taking four to twelve weeks from enrollment to certification. The process involves completing a state-approved training program, passing a two-part competency exam, and getting listed on your state’s Nurse Aide Registry. Here’s what each step looks like in practice.
Meet the Basic Eligibility Requirements
Most states require you to be at least 18 years old, though some allow 16- or 17-year-olds to enroll with restrictions. You’ll need a high school diploma or GED in many states, though not all. Every state requires a criminal background check before you can work in a healthcare facility.
Certain criminal convictions can permanently disqualify you. Felony homicide, any offense involving unlawful sexual behavior, sex offender registry status, and crimes related to selling or distributing controlled substances are common lifetime bars. Other offenses, including crimes of moral turpitude like public lewdness, typically disqualify you only if they occurred within the past seven years. If you completed a deferred adjudication agreement, you generally won’t be disqualified. Rules vary by state, so check your state’s specific list before investing in training.
Choose a Training Program
Federal law requires a minimum of 75 clock hours of training, including at least 16 hours of supervised hands-on practice where you demonstrate skills on a real person under the direct supervision of a registered nurse or licensed practical nurse. Many states exceed this minimum significantly. California, for example, requires 160 hours. Your state’s health department website will list the exact requirement and approved programs near you.
You have several options for where to train:
- Community colleges are typically the most affordable, with tuition ranging from $500 to $1,500. Programs often run 8 to 12 weeks and may offer financial aid.
- Vocational and private schools vary widely in cost, from $400 to $1,700 or more. Some include extras like job placement assistance, but check what’s actually included before paying a premium.
- Nursing homes and healthcare employers frequently offer free training. Federal rules require nursing homes to cover the cost of CNA certification training for people working at their facility. If you get trained elsewhere but are hired within 12 months of certification, you may also be reimbursed for your training costs. These “earn while you learn” programs let you draw a paycheck during training, making them the most budget-friendly option by far.
When budgeting, add the cost of scrubs or uniforms, textbook fees, a background check, and the state exam fee on top of tuition. These extras can add $100 to $300.
What You’ll Learn in Training
CNA programs cover a core set of skills you’ll use every shift. Expect classroom instruction on infection control, patient rights, communication with the care team, basic nutrition, and how to recognize changes in a patient’s condition that need a nurse’s attention. The clinical portion is where you practice the physical skills: taking vital signs (blood pressure, pulse, temperature, respiration rate), assisting with bathing and dressing, safely transferring patients from beds to wheelchairs, positioning patients to prevent bedsores, and measuring fluid intake and output.
You’ll also learn how to respond to emergencies, provide range-of-motion exercises, care for patients with catheters, and document your observations accurately. These aren’t abstract lessons. Instructors will watch you perform each skill and correct your technique before you ever touch a patient in a real facility.
Pass the Competency Exam
After completing your training program, you’ll take a state competency evaluation that has two parts: a written (or oral) knowledge test and a clinical skills demonstration. The written portion is multiple choice, covering the topics from your coursework. The clinical skills test requires you to correctly perform a randomly selected set of hands-on tasks in front of an evaluator, typically a nurse. Common tasks include handwashing, measuring blood pressure, making an occupied bed, and providing perineal care.
You usually need to pass both parts to earn certification. Most states give you three attempts. If you fail after three tries, you’ll typically need to retake the training program before testing again. Your training program should prepare you thoroughly, but practice each skill until it feels automatic. The most common reason people fail the clinical portion is skipping a step in the procedure, not a lack of knowledge.
Get Listed on Your State Registry
Once you pass the exam, your name goes on your state’s Nurse Aide Registry. This is what employers check to verify your credentials. Without an active registry listing, you can’t work as a CNA in a Medicare- or Medicaid-certified facility.
Keeping your certification active requires renewal every 24 months. The renewal requirement is straightforward: you need to document that you worked at least one day (eight hours) performing nursing or nursing-related services within each 24-month period. Some states also require a small number of continuing education hours. If your certification lapses because you haven’t worked, you’ll likely need to retake the competency exam to get relisted.
Moving Your Certification to Another State
CNA certification does not automatically transfer between states, but most states offer a reciprocity process that lets you skip retraining. The general requirements are consistent: you need to be listed in active, good standing on your current state’s registry, have no findings of abuse, neglect, or misappropriation of property, and show proof that you’ve worked as a CNA within the past two years or recently passed a competency exam.
Some states have agreements with neighboring states that simplify the process further. North Carolina, for instance, has a second reciprocity pathway specifically for CNAs moving from Georgia, South Carolina, Tennessee, Virginia, or West Virginia. The application typically involves filling out a form, authorizing a background check, and waiting for your new state to verify your registry status. Processing times range from a few weeks to a couple of months.
Job Outlook and Pay
The median annual wage for nursing assistants was $39,530 as of May 2024, according to the Bureau of Labor Statistics. That works out to roughly $19 per hour. Pay varies considerably by setting and location. CNAs in hospitals and metropolitan areas tend to earn more than those in rural nursing homes. Picking up evening, night, or weekend shifts often comes with a differential that bumps your hourly rate.
Employment growth is projected at 2 percent from 2024 to 2034, which is slower than average. But that number is misleading in isolation. Because the CNA workforce is large and turnover is high, the BLS projects roughly 73,000 openings per year from people leaving the field or retiring. Finding a job is rarely the hard part.
Many CNAs use the role as a stepping stone. Working as a CNA gives you direct patient care experience that strengthens applications to nursing school, physical therapy assistant programs, or other healthcare careers. Some employers offer tuition assistance for CNAs pursuing further education, which is worth asking about during interviews.
