How to Become a Certified Home Health Aide

Becoming a certified home health aide (HHA) is one of the fastest paths into healthcare, with most training programs taking just one to four weeks to complete. The federal minimum is 75 hours of combined classroom and hands-on training, though some states require more. Here’s what the full process looks like, from enrollment to certification.

Federal Training Requirements

The Centers for Medicare and Medicaid Services sets the baseline: all HHA training programs must include at least 75 total hours, with a minimum of 16 hours of classroom instruction followed by at least 16 hours of supervised practical training. The remaining hours can be split between classroom and clinical work depending on how your state or program structures the curriculum.

These 75 hours are the federal floor. Seventeen states and the District of Columbia exceed that minimum, while 33 states stick to exactly 75 hours. California, for example, requires 120 hours with at least 20 clinical hours. New York and Texas follow the federal standard of 75 hours with 16 clinical hours. Before enrolling in any program, check your state’s health department website to confirm the requirement where you plan to work.

What You’ll Learn in Training

HHA programs cover a wide range of personal care and safety skills. The core curriculum typically includes bathing (bed, tub, and shower), dressing, grooming, oral care, toileting, meal preparation, and feeding. You’ll learn proper handwashing technique following CDC guidelines, standard precautions for infection control, and how to recognize and report pain by noting its location, duration, intensity, and triggers.

Beyond basic hygiene tasks, training goes into several body systems and the care skills tied to each:

  • Skin care: identifying pressure points, early signs of skin breakdown, and how to give a proper back rub to promote circulation.
  • Urinary care: assisting with bedpans, bedside commodes, incontinence management, and measuring urinary output.
  • Nutrition and hydration: proper feeding techniques and positioning, measuring fluid intake, and recording weight using balance or chair scales.
  • Mobility: safe body mechanics for transfers and repositioning, assisting with walking using canes or walkers, and performing range-of-motion exercises (active, passive, and resistive).
  • Equipment: operating a hydraulic lift (Hoyer lift) and using a slide board for transfers.

You’ll also learn environmental care like making occupied and unoccupied beds, organizing a client’s living space, and in some programs, basic infant care including handling, bathing, and feeding. One detail that surprises many new students: HHAs can file fingernails and toenails but are generally not permitted to cut or trim them.

How Long the Process Takes

In states that follow the 75-hour federal minimum, some programs compress training into a single week of full-time attendance, covering 40 hours of classroom instruction and clinical practice. Other programs spread the same hours over two to four weeks with part-time or evening schedules. States with higher hour requirements, like California’s 120-hour standard, typically run closer to three or four weeks.

After completing the training, you’ll need to pass a competency evaluation before you can work. Some states issue certification within days of passing; others involve a registration process that adds a week or two. From the day you start class to the day you hold your credential, most people are looking at two to six weeks total.

The Certification Exam

The competency evaluation has two parts: written and practical. The written portion is a multiple-choice test covering basic patient care, safety and emergency procedures, patient rights, and communication with clients and families. The practical portion requires you to demonstrate hands-on skills in front of an evaluator. Expect to be tested on tasks like bathing, grooming, feeding, checking vital signs, and assisting with mobility.

Both sections must be passed to earn certification. If you fail one part, most states allow you to retake it without repeating the entire training program, though policies vary. Your training program will typically tell you exactly which skills are eligible for testing so you can practice them beforehand.

Background Check Requirements

Every state requires some form of background screening before you can work as an HHA. The process typically involves submitting fingerprints for a criminal history check through your state’s department of justice. In California, any conviction beyond a minor traffic violation requires a criminal record exemption before you can be present in a care facility. Certain serious offenses, including robbery, sexual battery, child abuse, elder abuse, arson, and kidnapping, are permanently disqualifying with no exemption available.

Other states have similar lists of disqualifying convictions, though the specifics vary. Background checks are usually submitted during or immediately after training, and results can take anywhere from a few days to several weeks depending on the state.

Keeping Your Certification Active

Certification isn’t a one-time event. To maintain your HHA credential, you’ll need to meet annual requirements. In Indiana, for example, HHAs must work at least eight hours in nursing-related duties and complete 12 hours of in-service training each year. Those with specialized credentials, like tube feeding certification, need an additional hour of annual training specific to that skill. Renewal is typically free and handled through your state’s online licensing portal.

If you let your certification lapse by not meeting the work or training minimums, most states require you to retake the competency evaluation or, in some cases, repeat the full training program. Staying current with your annual in-service hours is far easier than starting over.

How HHAs Differ From CNAs and PCAs

Three entry-level care roles often get confused: home health aides, certified nursing assistants (CNAs), and personal care assistants (PCAs). The biggest differences come down to training depth and clinical scope.

CNAs complete state-approved programs that are generally longer than HHA training and include anatomy, clinical skills, and infection control. They must pass a state competency exam and can perform tasks like taking vital signs and assisting with certain clinical procedures, usually in facilities like nursing homes or hospitals. HHAs receive shorter, more focused training centered on personal care and daily living support, and they work primarily in clients’ homes. PCAs have the most flexible path, often completing only employer-based training with no formal license. Their role centers on companionship, daily assistance, and engagement rather than hands-on medical care.

If you’re already a CNA, many states will recognize your credential as meeting HHA requirements with little or no additional training. Moving in the other direction, from HHA to CNA, typically requires enrolling in a separate CNA program and passing that state exam.

Pay and Job Outlook

The median pay for home health and personal care aides was $16.12 per hour, or about $33,530 per year, based on May 2023 data from the Bureau of Labor Statistics. Pay varies significantly by state, metro area, and employer. Aides working for Medicare-certified home health agencies or in high cost-of-living areas tend to earn more than the median.

Demand for HHAs is strong and growing, driven by an aging population and a broad preference for home-based care over institutional settings. Many agencies offer signing bonuses, flexible scheduling, and tuition reimbursement for further training as incentives to attract workers in a competitive labor market.
