This increased pressure is often referred to as elevated central venous pressure (CVP) or jugular venous pressure (JVP). The appearance of the distended vein can resemble a rope or raised tube beneath the skin’s surface. This phenomenon is not painful itself but acts as a visual indicator of internal circulatory dynamics.

How Healthcare Professionals Assess JVD

The patient is typically positioned in a semi-recumbent position, often with the head of the bed elevated to approximately a 30 to 45-degree angle. This angle allows the jugular vein pulsations to be most visible and measurable. The examiner usually stands on the patient’s right side, as the right internal jugular vein offers a more direct reflection of central venous pressure.
The assessment involves observing the neck for the highest point at which the jugular vein pulsation can be seen. A ruler is then used to measure the vertical distance from this highest point to a reference point on the chest, such as the sternal angle (Angle of Louis). This measurement, combined with an estimated distance from the sternal angle to the right atrium, helps estimate the jugular venous pressure. It is important to distinguish these venous pulsations from arterial pulses, as jugular pulsations are typically non-palpable, have multiple waves, and change with respiration.
