Why the 4-Hour Rule Exists

How to Sleep the First Night
Once you’ve passed the 4-hour mark, lying down is fine. But for the first night, sleeping on your back is the safest choice. This keeps pressure off your face entirely and lets the product continue settling into the injection areas without interference. If you’re naturally a side or stomach sleeper, propping an extra pillow on each side of your head can help keep you from rolling over during the night.
By the next morning, the toxin has had enough time to fully anchor into the nerve terminals. At that point, your sleeping position no longer matters.
Other Activities That Follow the Same Rule
The 4-hour window isn’t just about lying down. Any position that puts your head below your heart or adds pressure to your face carries the same migration risk. That includes bending over to pick things up, doing yoga, wearing tight headbands or helmets, and even vigorously wiping sweat off your forehead.
Exercise has its own set of concerns beyond just position. Strenuous workouts raise your heart rate and increase blood flow, which can disperse the toxin away from the injection site faster than it can bind. Most cosmetic clinics recommend waiting a full 24 hours before intense exercise like running, weight lifting, or cycling. Light activity like a gentle walk is generally fine after the initial 4-hour window, but anything that gets you breathing hard or sweating heavily should wait until the next day.
Heat exposure follows the same logic. Saunas, hot tubs, and even very hot showers increase circulation to your face, so it’s best to keep things lukewarm for the first 24 hours.
