A single dose of Cialis (tadalafil) can remain effective for up to 36 hours, which is why it’s sometimes called “the weekend pill.” That’s significantly longer than other erectile dysfunction medications, which typically work for 4 to 6 hours. But “up to 36 hours” doesn’t mean you’ll have an erection for a day and a half. It means you’ll have a window during which the drug can help you get an erection when you’re sexually aroused.
How Quickly It Starts Working
Most men notice effects within 30 minutes to 2 hours after taking a dose. In a clinical trial using stopwatch timing, 52% of men taking the 20 mg dose were able to have successful intercourse within 30 minutes. The reason Cialis works for up to 36 hours comes down to how slowly your body clears it. The drug’s half-life (the time it takes for half the dose to leave your system) is about 17.5 hours in most men. That’s roughly three to four times longer than sildenafil (Viagra) or vardenafil (Levitra), which is why those drugs wear off in 4 to 6 hours while Cialis keeps working well into the next day.

As-Needed vs. Daily Dosing
Cialis comes in two different dosing strategies, and they change the answer to “how long does it last” quite a bit.
As-needed dosing uses a 10 mg or 20 mg tablet taken before anticipated sexual activity. The recommended starting dose is 10 mg, which can be adjusted up or down. You take it when you need it, no more than once per day, and it provides that 24-to-36-hour window of effectiveness.
Daily dosing uses a much smaller dose, either 2.5 mg or 5 mg, taken at the same time every day regardless of when you plan to have sex. After about 5 days of daily use, the drug reaches a steady level in your bloodstream that’s roughly 1.6 times higher than a single dose would produce. At that point, it’s always active in your system, so there’s no need to plan around a pill. This approach is also used for men who have both ED and an enlarged prostate, since tadalafil treats both conditions.
Factors That Shorten or Extend the Duration
Age
If you’re over 65, the drug stays in your system longer. Older men have about 25% more overall exposure to the drug, and the half-life extends by roughly 5 hours (to about 21.6 hours instead of 17.5). That means it lasts longer but also means side effects can linger.
Kidney Function
Reduced kidney function has the biggest impact on how long Cialis stays active. Men with mild or moderate kidney impairment have roughly double the drug exposure compared to men with healthy kidneys, and the half-life can stretch to 22 to 26 hours. If you have kidney problems, your doctor will typically start you on a lower dose.
Liver Function
Liver impairment has a surprisingly small effect. Studies found negligible changes in overall drug exposure in men with liver problems, though the half-life may extend slightly.
Food and Alcohol
One practical advantage of Cialis over some other ED medications: food doesn’t significantly affect how the standard tablet is absorbed. You can take it with or without a meal and expect similar results. This isn’t the case with all formulations (dissolving film versions, for example, are affected by food), but the standard pill you’d pick up at a pharmacy is diet-independent.
Alcohol in moderate amounts won’t block the drug from working, but it can lower blood pressure on its own. Since Cialis also lowers blood pressure slightly, combining the two can increase the chance of dizziness or lightheadedness.
Common Side Effects and How Long They Last
Because Cialis stays in your body so long, side effects can also persist longer than with shorter-acting medications. The most common ones include headache, indigestion, back pain, muscle aches, flushing, and a stuffy nose. Back pain and muscle aches are somewhat unique to tadalafil and typically show up 12 to 24 hours after a dose, then resolve within a couple of days.
A rare but serious concern is priapism, an erection that won’t go away on its own. If an erection lasts more than 2 hours and becomes painful, that requires emergency medical attention. Priapism is uncommon with any ED medication, but the long duration of Cialis means the risk window extends further than it would with a shorter-acting drug.
The Practical Timeline
Here’s what a single as-needed dose looks like in practice. You take the pill and may notice effects within 15 to 30 minutes, though 1 to 2 hours is more typical for full effect. The drug peaks around the 2-hour mark. Over the next several hours, it gradually declines but remains effective enough to support erections with sexual stimulation for most of the next day. By 36 hours, most men find the effect has faded, though trace amounts of the drug remain in your system for longer than that (it takes about 4 to 5 half-lives, or roughly 3 to 4 days, for it to fully clear).
