For those with male anatomy, the most common collection method is a first-catch urine sample. To ensure accuracy, you may be instructed to avoid urinating for one to two hours before the test.
For those with female anatomy, a vaginal or cervical swab is often used, and this can sometimes be self-collected. If the infection is suspected in other areas, such as the throat or rectum, swabs may be taken from those sites. The actual time spent in the clinic for the collection itself is usually brief, often taking less than five minutes.
Understanding Result Turnaround Times
The most significant waiting period is the time required for the laboratory to process the specimen and deliver the results. Most gonorrhea tests rely on Nucleic Acid Amplification Tests (NAATs), which detect the bacteria’s genetic material. In a standard clinical setting, NAAT results are usually available within one to five business days after the sample reaches the lab.
Many labs aim for a turnaround time of 24 to 72 hours, but this can be affected by factors like the volume of tests being processed, the location of the testing site, and whether the sample needs to be shipped. A specialized option is a rapid or point-of-care test (POCT), which processes the sample on-site at the clinic. These newer NAAT-based POCTs can deliver results in as little as 30 to 90 minutes, allowing for immediate treatment before the patient leaves the facility.
Timing the Test Correctly
Waiting for the correct biological time, known as the “window period,” is crucial for an accurate result. The window period is the time between potential exposure and when the infection can be reliably detected by a test. Testing too soon may lead to a false negative result, meaning the infection is present but undetectable.
For gonorrhea, the window period is typically about two weeks post-exposure for urine and genital swab tests. While NAATs may detect the infection earlier, waiting the full two weeks is recommended for maximum accuracy. If symptoms are present, testing should be sought immediately, regardless of the time since exposure.
Next Steps: Treatment and Follow-up Testing
If a test result is positive, the next phase begins with prompt treatment, which is typically a single injection of an antibiotic. Following this single-dose treatment, the infection usually clears up within seven to 14 days. It is important to abstain from sexual activity for a full seven days after receiving treatment and until any symptoms have completely resolved, to prevent spreading the infection.
The timeline also includes follow-up testing to ensure the infection is completely gone or to check for reinfection. For uncomplicated genital or rectal infections, a “Test of Cure” is generally not needed if the recommended antibiotic regimen is used. However, retesting is strongly recommended three months after treatment to check for possible reinfection, which is common if a partner was not also treated. For pharyngeal (throat) infections, a Test of Cure is recommended seven to 14 days after treatment to confirm the bacteria has been eradicated.
