How Long Can Ozempic Be Left Unrefrigerated?

Once you’ve started using an Ozempic pen, it can stay unrefrigerated for up to 56 days (8 weeks) as long as the temperature stays between 59°F and 86°F. After 56 days, the pen should be discarded whether or not medication remains inside. Unopened pens, however, need to stay refrigerated until you’re ready to use them.

Storage Rules for New vs. Opened Pens

The rules differ depending on whether your pen has been used yet. Before first use, Ozempic must be kept in a refrigerator between 36°F and 46°F (2°C and 8°C). This is non-negotiable for unopened pens. Don’t store them in the freezer or near the cooling element inside your fridge, because freezing destroys the medication. If an Ozempic pen has been frozen, it should not be used.

After your first injection from a pen, you have more flexibility. The pen can be stored either back in the refrigerator or at room temperature, anywhere from 59°F to 86°F (15°C to 30°C). The 56-day clock starts from the first use regardless of where you store it. Keep the pen cap on when it’s not in use, and keep the pen away from excessive heat and direct sunlight.

What Counts as “Too Hot” or “Too Cold”

The safe room temperature range tops out at 86°F. That number matters because it’s easy to exceed in everyday situations. A car parked in the sun can reach well over 100°F in minutes, even on a mild day. A pen left in a hot car, sitting on a sunny windowsill, or packed in checked luggage exposed to cargo hold temperature swings may be compromised. The FDA-approved label specifically warns against excessive heat and sunlight.

On the cold end, anything at or below freezing is a problem. Freezing damages the protein structure of semaglutide, the active ingredient, and there’s no way to restore it. If you suspect your pen froze, even briefly, don’t use it.

Traveling With Ozempic

Travel is the most common reason people worry about refrigeration. A few practical strategies keep your pen safe:

  • Use an insulated travel case. A simple medical cooling pouch maintains temperature for hours without electricity. Just make sure the pen doesn’t directly touch any ice packs, since direct contact can freeze the medication.
  • Carry it on, don’t check it. If you’re flying or taking a train, keep Ozempic in your carry-on bag. Checked luggage goes into cargo holds where temperatures can swing dramatically.
  • Keep it in the cabin of your car. The trunk tends to get significantly hotter or colder than the passenger area. And never leave the pen in an empty parked car for extended periods.
  • Refrigerate unopened pens at your destination. If you’re bringing spare pens you haven’t started yet, get them into a fridge as soon as you arrive.

How to Tell if Your Pen Is Still Good

Ozempic solution should be clear and colorless. If the liquid looks cloudy, has visible particles, or has changed color, don’t use it. These are signs the medication has degraded, potentially from temperature exposure. That said, a pen that was left out at room temperature for a few hours within the 59°F to 86°F range is well within the approved 56-day window and is fine to use.

If your pen was briefly exposed to temperatures above 86°F, there’s no reliable way to test its potency at home. The safest approach is to contact your pharmacist, describe the exposure (how hot and how long), and ask whether the pen should be replaced. Short, minor excursions above the threshold are different from hours in a hot car, but there’s no published guidance drawing a precise line.

The 56-Day Rule in Practice

Most people using Ozempic inject once per week, and a single pen typically contains enough medication for multiple doses depending on the prescribed amount. At higher doses, you may use a pen within a few weeks. At lower starting doses, the pen could last longer, but it still must be discarded after 56 days from first use, even if medication remains. Marking the date of first use on the pen or setting a calendar reminder helps you track this window so you’re not relying on guesswork.
