How Is Semaglutide Administered: Shots and Oral Use

Semaglutide is available in two forms: a once-weekly subcutaneous injection and a daily oral tablet. The injectable versions are sold as Ozempic (for type 2 diabetes) and Wegovy (for weight management), while the oral tablet is sold as Rybelsus (for type 2 diabetes). Each form has specific requirements that affect how well the medication works.

Injectable vs. Oral Forms

Both injectable brands, Ozempic and Wegovy, use pre-filled pens that you inject under the skin once per week. You pick a day of the week that works for you and stick with it, though you can shift by a day or two if needed. The injection doesn’t need to be timed around meals.

Rybelsus, the oral tablet, is a daily medication with stricter rules. You take it first thing in the morning on an empty stomach with no more than 4 ounces of plain water. Then you wait at least 30 minutes before eating, drinking anything else, or taking other medications. These requirements exist because food, beverages, and even the volume of water you use can interfere with absorption. Waiting less than 30 minutes or taking the tablet with food significantly reduces how much of the drug your body actually absorbs. Waiting longer than 30 minutes can increase absorption.

Where and How to Inject

The recommended injection sites are the abdomen, the front of the thigh, and the upper arm. All three areas have enough subcutaneous fat for proper absorption. You should rotate between these sites each week to prevent skin irritation and scar tissue buildup. Scar tissue at a repeatedly used site can block the medication from absorbing effectively.

The pens come with very thin, short needles: 32-gauge and just 4 mm long. That’s one of the thinnest needles available for injectable medications, which makes the process relatively painless for most people. You press the pen against your skin and hold the dose button down until the counter reads zero.

Preparing the Pen

Before your first injection with a new pen, you need to do a flow check. This confirms that the pen is working and clears any air from the needle. You attach a new needle by pushing and twisting it on until tight, remove both needle caps, then turn the dose selector to the flow check symbol. Press and hold the dose button until the counter shows zero, and look for a small drop of medication at the needle tip. If no drop appears, you repeat the process up to six times before using a different pen.

Before each use, check that the liquid inside the pen is clear and colorless. Cloudy or discolored medication should not be used. You attach a fresh needle for every injection.

Dose Titration Schedule

Semaglutide isn’t started at the full therapeutic dose. Both the injectable and oral forms follow a gradual increase, called titration, designed to reduce nausea and other gastrointestinal side effects.

For weight management with Wegovy, the schedule works like this:

  • Weeks 1 through 4: 0.25 mg once weekly
  • Weeks 5 through 8: 0.5 mg once weekly
  • Weeks 9 through 12: 1 mg once weekly
  • Weeks 13 through 16: 1.7 mg once weekly
  • Week 17 onward: 2.4 mg once weekly (maintenance dose)

Each step lasts four weeks before increasing. If you experience significant nausea or other side effects at a new dose, your prescriber may hold you at that level for an additional four weeks before moving up. If the full 2.4 mg maintenance dose causes persistent problems, a temporary step back to 1.7 mg for four weeks is an option before trying again.

Ozempic for diabetes follows a similar pattern but with different target doses, typically reaching 0.5 mg, 1 mg, or 2 mg depending on your blood sugar response.

What to Do if You Miss a Dose

For the weekly injection, take the missed dose as soon as you remember, as long as your next scheduled dose is at least two days away. If it’s closer than that, skip the missed dose and resume your regular schedule. Never take two doses to make up for a missed one.

For Rybelsus tablets, if you miss a morning dose, skip it and take the next one the following day as usual. Taking it later in the day after you’ve already eaten defeats the fasting requirement.

Storage Requirements

Before first use, both Ozempic and Wegovy pens need to be refrigerated between 36°F and 46°F. Once you start using a pen, or if it’s been outside the fridge, the clock starts ticking. Ozempic pens are good for 56 days at room temperature (59°F to 86°F). Wegovy pens are more sensitive, lasting only 28 days outside refrigeration and tolerating a slightly wider temperature range of 46°F to 86°F. Neither should be frozen, and pens that have been frozen should be discarded.

Rybelsus tablets are simpler to store. They stay at room temperature (68°F to 77°F, with brief excursions up to 86°F allowed) and remain good until their expiration date. Keep them in the original blister packaging, since moisture can degrade the tablet.
